The product, in the report: LED Lightbulb (800 lumins)

The light was in use for approximately 2 hours, when part of the glass bulb separated from the kitchen, pendant ceiling light fixture.

The consumer heard a noise and went to check on it. Part of the light bulb’s glass separated, shot down on the counter and landed on the floor. The part that hit the floor remained intact.

The round part of the bulb that the light comes out of is plastic. However, the metal portion was extremely hot to the touch when the consumer felt it. If this part had landed on a piece of paper or something flammable, he believes that it could have easily caught fire. Additionally, part of the bulb that remained in the light fixture emitted smoke for approximately 5 minutes. There were no injuries.

The consumer turned the light off and removed the metal portion. The consumer has never had any problems with the fixture in the past.

The consumer conducted an internet search, but the firm appears to either have been sold or to have cease making residential lighting products.

The product, in the report: Osram 4’ LED bulb - Model # 73314-0 Description LED19T8L48/F/841/SUB T8 replacement

We currently are using an Osram brand, 4’ LED bulb that was used to replace a fluorescent bulb in a fixed ballast. This was done under a program through NYSEG/RG&E in 2014/2015. Our bulbs were all placed toward the end of 2014 and beginning of 2015. Recently (approximately 2 weeks ago), we started to notice that some of the bulbs had burned out. Being an LED we found this odd that these bulbs would burn out so quickly. Upon further inspection, it was found that the bulbs had actually melted inside of the fixture. Each bulb that had burned out varied in severity, but some were bad enough that they actually burned and charred the ballast, as well as the ceiling that the ballast was attached to. The bulbs themselves have melted, and appeared to have caught fire. I still have the affected bulbs. I contacted the company and was told that there was actually a recall done on the bulbs, however my bulbs were not part of the recall. I was told that the recall included model numbers [REDACTED and [REDACTED. Ours are 73314-0. Each day that goes by, we are losing more bulbs to this. We are up to a total of 12 currently, and we started with 6 two weeks ago. The bulbs have actually got hot enough to damage the actual ballast and make it inoperable. The company states they can warranty the bulb, but that is it. Nothing can be done further because it was not part of the recall. We have a total of 142 of these bulbs in our ambulance station, so as you can see Im very concerned that these will continue to burn up with a high risk of a possible serious fire or injury.

The product, in the report: LED19T8/L48/F/835/SUB model 73313-0

I had completed a LED retrofit replacement of approx. 200 tube lights in a retirement home. Two of the bulbs had complete melting at the ends. The plastic had dripped onto the light lens and also damaged the fixtures at the contact ends. I worked with Nedco in Mississauga Ontario for the retro fit program. When looking up the Osram bulbs on line, they are made by Ledvance. The two models that were recalled were [REDACTED] and [REDACTED] My model number is 73313-0, which did not have a recall. The on-line lamp recall information said to call Ledvance. My contact there has been [REDACTED]. The first bulb that melted was returned for product testing and the second bulb that was discovered on Jan 5, 2018 was not retuned but [REDACTED] was notified. Ledvance replaced all the LED bulbs at my request. I was given direction by [REDACTED] to purchase 2 new fixtures from Nedco and have them installed. I would be reimbursed for the costs. I had this completed and [REDACTED] had [REDACTED] at XL Specialty Insurance Company contact me. I sent her my invoices as she requested. [REDACTED] contacted my and said she would email me the release and when it is returned the cheque would be issued. A couple emails have gone back and forth regarding the release as this was not disclosed until after I submitted my invoices as instructed. I feel at this point the company went ahead with instructing me to spend money on the replacements and is now refusing to make payment as they require a release of liability to make the payment. Not only does the product create a huge safety concern but Ledvance would like to remove themselves form the liability.

This lightbulb (less than 2 years old) was being used in my bathroom lighting fixture (an over the mirror 2 light fixture which is less than 2 years old). It had been on for several hours and started flickering. It kept getting worse, so I turned off the light and removed the bulb. I expected it to be hot, so I was very careful with it, however I received second degree burns on the pads of my index finger and thumb when I lightly touched the ballast. I put the light bulb down to care for my burns and came back It kept getting worse, so I turned off the light and removed the bulb. I expected it to be hot, so I was very careful with it, however I received second degree burns on the pads of my index finger and thumb when I lightly touched the ballast. I put the light bulb down to care for my burns and came back to it several minutes later to see if it was still hot. I put a large drop of water on the ballast, and within 4 seconds it was boiling. 20 minutes later the bulb was still to hot to pick by the ballast. The bathroom was moist, due to a long shower, but a fan venting to the outside was running.

This LED bulb was purchased at Lowes and installed into a 3 light ceiling fan in our bathroom approximately 3 months prior to incident date. While my wife was in the shower, she heard a loud pop and the lightbulb fell to the floor. It did not break, but a good deal of smoke was observed in the fixture area when I got into the bathroom. The base of the bulb was exposed with some visible melting noted on the plastic base adjacent to the little diode as well as the plastic covering of the one wire. The base was extremely hot to touch. The metal part of the base remained in the light fixture.

We purchased this bulb, along with 8 others, about a year ago for a large hanging lighting unit in our living room. We knew that it would save a tremendous amount of energy to replace 9 60W bulbs with LED.

Today, however, we heard a loud pop sound, and one of the bulbs blew completely out of the socket, flew up about 5 feet hitting the ceiling, and fell onto one of our chairs trailing black smoke.

The globe part of the bulb was not hot, but the base was hot enough to cause injury, and smoked for a while after coming to rest.

If you look at the picture, you can see a part of the plastic that melted.

We normally turn off the lights when we are not here, but it is certainly possible that no one might have been in the room when this occurred, which could have resulted in a fire.

On 10/7/16 during the evening I observed one of the Led bulbs in the ceiling fan flickering in our kitchen ceiling fan. I turned the light off & tried removing the bulb from it’s socket. The bulb cover sealed to the ceramic base separated. When I attempted to remove the Led from the socket it was very hot & caused slight burning of my left fingers. I finally removed the Led from the socket. On a previous night one of these bulbs stopped working for several minutes then went back on. This has also occurred in my living room lamps, the bulb would stop lighting & then turn back on. I have changed over 95% of my home to Led bulbs, bulbs in the past year. I’m concerned about the safety of these bulbs. As I inspected the ceramic part of the bulb it appears that only a bead of silicone is used to attach the bulb cover to the Led base. These bulbs are not reliable.

Purchased a Sylvania OSRAM LED light bulb from Lowes and installed in a 5 light ceiling light fixture with glass shrouds. The fixture is open at the top providing access to insert and remove a bulb. I consider this to be a non-enclosed light fixture. The bulb bears the following specifcations: OSRAM 8.5 W LED A19 5000K E26 5000K 800 lumens 8.5 W 120V 60Hz 77 mA. On Monday 18 July 2016, while seated at our dining table we noticed a flicker or surge from that light fixture. This was soon followed by a loud and smoke as the top part of the Sylvania LED bulb blew away from its metal screw-in base. The bulb landed on our dining room table. The bulb remained intact. The metal screw-in portion of the bulb was easily removed from the fixture. The base of the bulb – e.g. heat sink (ceramic material ?) remained very hot for several minutes. We consulted the following web site: http://www.saferproducts.gov/. We noted 56 similar complaints filed by consumers. In our opinion, this product is not safe and may have not been subjected to rigorous testing. We recommend other consumers contact the manufacturer and file similar reports. We will not use this bulb in our home.

I purchased 5 OSRAM A19 Bulbs about 1.5 months ago from Lowe’s - on sale. about 1 month ago i installed one in a table lamp replacing an old bulb that was not LED. Yesterday (10/13/2015) this bulb became a missile. The Glass bulb part had separated from the metal screw in part and flew up into the lamp’s shade then ended up on the floor behind the nightstand - glass totally intact, but very very hot. I call Sylvania yesterday and they were supposed to get back to me on this with hopefully a resolution. I am afraid to use any of these bulbs! This could have started a fire or injured someone. Please let me know what I should do now. I still have the bulb and used pliers to remove the metal part of the bulb from the lamp - It looks as though something melted in the base thanks

I had a OSRAM 8.5W LED Bulb in a bathroom fixture. Last night while the light bulb was on the bulb exploded. The Bulb Popped and ejected from the fixture with some force. The bulb and heat sink separated from the metal contact. A large amount of smoke was generated. The bulb did not shatter.

The product, in the report: LED Light Bulb

A few months ago I purchased two OSRAM LED 8.5 watt bulbs at Lowe’s Home store. One was installed in a reset lighting fixture in the kitchen. The one in a floor lamp.

I chose these bulbs as an alternative to incandescent bulbs. Both bulbs were on for four, six or eight hours at a time. Occasionally, the one in the reset fixture was left on over night. Three days ago, the bulb in the reset fixture, exploded, emitting black smoke, causing me to think a fire may have started.

When I removed the bulb, it was extremely hot to the touch. . .not at the bulb dome which was plastic, but at the metal connection to the socket. I saw that it had exploded, or separated from its base.

I immediately check the bulb in my floor lamp and it too was overheating at the bulb base. These bulbs are a true fire hazard and I don’t feel they should be on the market.
