The product, in the report: LUMAcoil CFL Model L30123T2 23W 120V60Hz 420mA 2700K
This CFL bulb was in a basic ceiling fixture in our basement. No dimmer is attached. My husband was in the basement with the lights on. This bulb started to flicker and he was looking at it. It made a small pop and a flame came out. Luckily he was there to remove the bulb before the fire could spread. I do have pics of the bulb.

The product, in the report: Consumer Light Bulb I have photos but the web site will not let me upload them.
The light is used over our stove and is left on 24 hours a day 7 days a week. It has been in place for about 3 months. It is never turned off.
On Wednesday afternoon it exploded.
Glass shards were scattered up to 4 feet away and glass fell all over our stove and kitchen counters.
There was no warning. The light did not dim or make any noise.
There were no visible power fluctuations. I was home at the time working on my computer and was within 20 feet of the light when it failed.

No one hurt Made Sep 22, 2012, sent to the company Oct 1, 2012, published by CPSC Oct 16, 2012
The product, in the report: Compact Fluorescent Light Bulb Lumacoil L30123M, 23W, 120V, 60Hz, 363ma “AM Conservation Group, Inc. Charleston, SC” printed on the base
Compact florescent bulb in table lamp exploded. The bulb started to flicker, so I turned the lamp off. I had forgotten about the bulb doing this and turned the lamp on that evening and nothing happened, no flickering at all. I turned the lamp off and went to get a replacement bulb and got about 5 steps away from the lamp when it exploded. Even with a lamp shade on the debris went out about 7-8 feet. The base of the bulb came apart and the circuit board is exposed. There is a burn mark on the plastic cap around the area where the glass tube enters the cap. I don’t know how long this bulb was in this lamp. It is one of the larger compact florescent bulbs and may be 5-7 years old. It is a Lumacoil L30123M, 23W, 120V, 60Hz, 363ma bulb. It has “AM Conservation Group, Inc. Charleston, SC” printed on the base. I have photos and have bagged the base and will keep for 2 weeks, if some one wants it. I have thrown away the glass.
