First aid Made Nov 19, 2017, sent to the company Feb 28, 2018, published by CPSC Mar 14, 2018
The product, in the report: GTC 72 watt (1490 Lumens) halogen clear light bulb
Using a GTC 72 watt bulb in bathroom fixture. Had noticed a dull residual glow from same bulb a couple nights before after power was turned off for about 30 seconds. Day off incident, light was used for approximately 10 minutes then turned off. About 5 to 10 minutes after being turned off, bulb exploded sending glass in about a 10 foot radius. Part of a filament had landed on a hand towel, which in turn caused a fire on the bathroom sink and wall. I received a minor burn on hand from putting fire out and had to remove 2 pieces of glass from my foot (was barefoot at time of incident).

Purchased a 65W halogen indoor floodlight at HEB in Atascocita TX on or about 5/14/2016. GTC, BR30 120V bar code 41220 98950. No one hurt Made Jul 20, 2011, published by CPSC Aug 10, 2011
The product, in the report: GTC compact fluorescent light bulb
Our GTC compact fluorescent light bulb went out and immediately after, we saw smoke coming from the socket. We quickly removed the light bulb to find that the bottom of the bulb had melted and turned black and was extremely hot. We went around the house to find any other GTC fluorescents and found one other in a similar condition (not melted, but black on the bottom) and removed all GTC bulbs and threw them away to prevent any possible fires.
