No one hurt Made Apr 28, 2022, sent to the company May 5, 2022, published by CPSC May 19, 2022
The product, in the report: 48” LED direct replacement bulb (T8 17w 5000k) for a florescent light fixture.
Last night I had a small fire / meltdown of a LED light bulb in a light fixture in my basement. I was able to extinguish the fire by removing the light bulb cover and removing the burning bulb, (then powering down the light fixture itself). A side room in my basement has become my temporary office and it was because of this that I witnessed first hand the beginning of the fire. The lights are activated by a motion sensor and shut down about five minutes later (if there is no motion). It was the end of this ‘on’ cycle that I seen something glowing orange on the basement floor in an adjunct room (right outside the door), the glow was melting/burning plastic of the fixture cover and one end of the bulb. *I’m concerned because I have installed an entire box of these bulbs in my house.

No one hurt Made Sep 4, 2019, sent to the company Sep 13, 2019, published by CPSC Sep 27, 2019
The product, in the report: Typical T8 LED with white plastic caps with writing. Replacement T8’s for CFL tube fixtures Labeled GT-Lite with model # GT T8 EBIR Labeled on the tube for proper use in fixtures with electronic instant on ballasts. (see photos) Labeled for light output ect. (see photos)
The T8 LED bulbs were being used in a light fixture in our laundry room. The LED bulbs were a direct replacement for the original CFL bulbs. They were installed about 1 year ago.
On 9/3/2019 at about 6:30 am the fixture erupted in flames that shot out one end. damaging the ceiling and destroying the fixture. I needed to use a fire extinguisher to put out the fire . The fire dept. was called to be sure the fire had not gotten into the attic. It melted the plastic cover over the floor and scorch the ceiling. The fixture was operated by a occupancy sensor.
The product was GT-Lite T8 LED direct replacement bulbs. Model number GT T8 EBIR
It is very obvious that the bulb itself failed in a catastrophic manner. It did not blow the circuit breaker as I had to turn off the power after I put the fire out. The house is 13 years old and up to code.

The product, in the report: LED replacement bulbs for fluorescent light fixtures
I’m a fire investigator with the Michigan State Police. I have investigated a fire where a new LED replacement bulb/tube was installed in an existing fluorescent light fixture following the listed manufactures instructions. The bulb was purchased from Menard’s approximately 3 weeks prior to the fire. The fire started in the end of the bulb/tube where the electronics are located. The bulb/tube has a sticker that indicates it was manufactured June/17. The bulb/tubes are, GT-Lite LED model# GT-2PK-T8-3IN1
I have an incident report and photos of the scene filed under my complaint number [REDACTED].
