The product, in the report: T8 LED fluorescent replacement bulb

Purchased 150 T8 LED bulbs to replace fluorescent bulbs In a commercial building in March 2019. These are designed for use with electronic ballasts. Approximately 1 year after installation one bulb exploded creating a significant amount of smoke. Then 3 months later a second bulb exploded with smoke, fire and shattered glass falling on a person below. Contacted the manufacturer (Greentek energy systems) who said that they had never heard of this problem and that the ballast model was not on the list of those they had validated. It would have been acceptable had the bulbs not functioned properly but this type of potentially very dangerous catastrophic failure is never acceptable.
