Made Mar 11, 2022, sent to the company Mar 16, 2022, published by CPSC Mar 30, 2022

    The product, in the report: SELF-BALLASTED LAMP 23W/ELS-M 2700K 120V 60HZ 380mA LIGHT BULB WITH UL # E176533

On 1/27/2022 morning around 6:30am I was in bathroom on toilet at my house when the Greenlite bulb that I had been using for around 3-4 years was flickering for a few second then went dark. I thought that was it. The bulb is dead and I just needed to replace a new one. Then to my surprise, the dead bulb started showing sparks and white smoke and the situation got intense really quick when fire showed up a few seconds later and the whole bathroom was filled toxic smoke. I had to move fast to remove the bulbs because it was on fire while switched off the power. When everything settled down, I already inhaled so much smoke that made me very uncomfortable with bad headache. The bathroom that the incident occurred at has never been used for bathing or taking shower. All our family members only go there for using the toilet and wash hands. Therefore, there is minimum humidity in the bathroom for the 20 years we have been living there and it was the first light bulb caught on fire I have ever seen in my life.

Luckily, I was in the bathroom at the right time to witness the whole thing. Otherwise, my house could have been on fire or burned down for sure. I contacted Greenlite and talked to the person in charge, [REDACTED], who is the director of sales, but he seemed not care about the incident at all, and tried to make the whole incident goes away by offering me a replacement bulb, which I denied the offer right away. He then said he is done talking to me and hang up the phone rudely and suddenly. I am not upset about his attitude, but have great concerns about the safety of this kind of light bulbs the company made and sold. I would like your organization to investigate how many other cases happened before, and prevent similar incidents from happening again in the future. After all, your action toward the said light bulb may save people’s lives eventually. Thank you for your attention and help.

The product, in the report: Self-Ballasted Lamp

The consumer indicates that his wife was in another room when she noticed the self-ballasted lamp in the kitchen had turned off spontaneously. The consumer’s wife came into the kitchen and noticed the unit was down in flames, and a distinct smell of burning was being emitted. The consumer’s wife proceeded to turn the fixture off, and remove the chimney from the self-ballasted lamp; she noticed that there was charring, as well as ashes. The consumer removed the bulb.

The consumer states that the self-ballasted lamp was turned on approximately two to three hours daily.

The consumer plans on keeping the self-ballasted lamp for at least the next thirty days.

This morning, I went to turn on my daughter’s light in her closet. It didn’t turn on and there was a bad smell. The CFL bulb looked like it had melted off of its base and had fallen to the floor. The base was still plugged in and it has black streaks stained going upwards on it. I will be attaching photos.

The product, in the report: GREENLITE CFL BULB

I noticed that a lamp in my living room seemed dimmer than usual.I went over to see if the bulb was going out and noticed that there was smoke coming from top of lamp shade. I looked under the shade at the bulb and there was the beginning of a flame coming from the base of the bulb. There was a very strong electrical smell also. I am concerned about the potential fire threat and plan on replacing all of these “Greenlite” brand bulbs with another brand in my home. I am informing all of my co-workers, family and friends not to use this brand. My teenage son is often alone and being a teenager I’m not sure he would have even looked up from his computer before the curtain next to the lamp would have caught fire.

I was in my living room when I smelled something like it was burning. I followed the smell with my nose until I finally found it. I had to look up at my light in my bedroom to see a CFL light bulb melted. It had melted and had burn spots on it. The smell was pretty strong, so I opened the window. I’m lucky it didn’t cause a fire, but it could very well have. Here is a link to the photo I took of it still attached to the socket: [REDACTED]

This morning we noticed smoke coming from our kitchen light. One of our GREENLITE 18W/ELS- M 2700K 120V 60Hz290mA was actually smoking / buring. We immediately turned off the light and pulled the bulb once it had cooled down. We were shocked to see the burn marks on the bulb where the glass enters into the fixture base. Our entire home has these types of bulbs and we are now terrified that these bulbs are defective and may cause a major fire hazard in our home!

The product, in the report: Last evening, September 14th, at approximately 6pm a lamp with a compact fluorescent bulb in our lower family room came on and started smoking. The lamp gave off a burning electronics smell and set off all of the smoke detectors in our home. My wife immediately disconnected the lamp. After we cleared the house of the smoke using our whole house fan, I examined the lamp and discovered that the compact fluorescent bulb had a circular burn hole approximately 1/2 inch in diameter in the base. We were fortunate that we were home at the time as we were about to leave to go out for dinner and this event could have caused a fire in the lower level of our home. I have since replaced the bulb with another by different manufacturer and the lamp works fine. The burned compact fluorescent bulb was made by Greenlite Lighting Corporation, 10 Corporate Park, #100, Irving CA 92606. Please contact me by email or phone at [REDACTED] if you require additional information or want the burned out bulb. Thank you. [REDACTED]

The product, in the report: CFL bulb, 60W equivalent

Greenlite compact fluorescent light bulb burned out by literally burning around its base. I saw actual sparks and small flames when it happened, and when it was done, the base was charred and there was a strong chemical smell permeating two floors of our house four hours.

I’ve actually seen this happen with several Greenlite CFL bulbs, but this was by far the worst case, so I decided to take pictures of the bulb and file this report, although it has taken me a while to find the time to do so.

I am concerned both about the potential for one of these bulbs to cause a fire when it burns out, and by the potential for toxic chemicals like mercury to be released into the air.

Yesterday was the second time that a Greenlite flourescent light bulb started to catch fire in our home upon burning out. Luckily, I was sitting under it when I heard it fizzle out. My husband and I looked up and saw sparks and smoke coming from the bulb and socket, plus we could hear sizzling sounds.

There was a strong odor that caused me to have an immediate negative reaction for which I had to take my asthma and allergy medicine.

My husband turned off the power and when the bulb had cooled, he removed it. This is the second time a Greenlite bulb has caught fire upon burning out. The bulbs are only 1 year old!

The ceiling fan has two compact fluorescent light bulbs in it and was operating for about 60 minutes when one of the light bulbs burned out. About 15 minutes later, I smelled a terrible electrical burning smell and my husband and I determined it was coming from the ceiling fan. He removed the globe and used gloves to remove the very hot fluorescent light bulb. When the bulb cooled offwe noticed the spiral tubes were at an angle because the base where they were connected had melted and had turned black. The air in the house smelled horrible for several hours. There was a scorch mark on the housing of the globe. If we hadn’t been home, it could have caused a fire and our house could have burned down.

Last night we started noticing a burning electrical smell in our kitchen area. We did not know where it was coming from so I turned off the lights and the electrical breakers and called an electrician. While waiting for the electrician to come out turned the breakers on and still no smell. Then when I turned on the lights I started to smell the burning electrical smell. It turns out that inside one of my hanging light fixtures I started to see a small amount of smoke. Not sure if there are any safety recalls about this bulb but just wanted to let you know about it. I believe I purchased it a Ralphs Supermarket Mira Mesa (San Diego, CA) in 2007.The bulb says E176533 Greenlite 23W/ELS-M/Z 2700K 120V 60HZ 390 mA T33.

    The product, in the report: GREENLITE 23W/ELS-M/A 2700K 120V 60Hz 330mA E 176533 FLUORESCENT SELF-BALLASTED LAMP 61Y6

On Friday, 06/03/2011 at approximately 7:00PM in the evening, my youngest daughter and I noticed the smell of “burning electronics” in our kitchen. I quickly turned off the stove, and a shout brought the rest of the family in to the kitchen to track downt he smells source.

We located the source. It was a CFL in our ceiling fan. We quickly turned off the ceiling fan lights and waited for everything to cool. I then inspected the lights and found that one of them had melted/burned at the point where the fluorescent tubes attach to the base. The base is normally an off white, but was now orange to purplish-brown, and clearly burned/melted.

The information printed on the base of the CFL is as follows: GREENLITE 23W/ELS-M/A 2700K 120V 60Hz 330mA E 176533 FLUORESCENT SELF-BALLASTED LAMP 61Y6

I inspected the other two CFLs in the ceiling fan. Anothe one of the same manufacturer/model was showing signs of burning/melting. The third CFL in the fan was a different manufacturer/model and had no signs of problems.

Inspecting the rest of the CFLs in the house, I found two other GREENLITE units (same mode) that were showing signs of discoloration at the base, turning orangish, but not the signs of melting yet.

The other CFLs in the (different manufacturers) did not have signs of this problem.

Additional Information: ------------------- These CFLS are not new. They have probably been installed for more than a year.

3 of the 4 CFLS were installed in ceiling fixtures so they where inverted, with the base at the top and the ‘bulb’ at the bottom.

I have digital pictures of the units, and will hold on to them for a few weeks in the event that you wish to analyze them.
