The product, in the report: LED lightbulb
A Great Value lightbulb exploded in my son’s room, shattering the thick glass cover of the light fixture and sending glass shrapnel all over his room. The lightbulb was a Great Value 8W LED bulb (LED8A19DIMCL85018Y). My son had this bulb in his ceiling light fixture in his room for over a year. The other day, he turned his light on and it immediately began to smoke and exploded a moment later. Luckily, none of the shrapnel hit him, but given the dispersion of glass throughout the room it very well could have.

The product, in the report: Great Value 48” LED 4100K 40W tube light bulbs (designed to be replacement for T12 CFL tubes)GVL4T812. All failures (at least 4; some were not photo or video documented) came from the same value pack multiple product box.
Multiple Great Value (Wal Mart brand)T8 LED replacement bulbs bought and installed to replace CFL tubes at the same time have failed and caught fire in normal use. First failure was 11/19/23. Several since then (up to attached video on 2/16/24) resulted in removal of all of these throughout my house - many. I don’t recall exactly when I installed all of them but it was at the same time and they have failed in the same manner at least 3 times, in different light fixtures. In all cases, bulbs began emitting electrical smoke when switched on; flashed and flamed; melted and sagged; 2 broke upon bending and shattered.

The product, in the report: LED Tube Style Bulb
[REDACTED] Fire Rescue Dept. responded to an incident where a fire had occurred in a light fixture of a multi-family assisted living facility. Upon arrival and after ensuring the fire was out it was determined that a bulb had overheated and caught the plastic lens of the light fixture on fire. Staff on location had used an extinguisher on the fire prior to the FD arrival. The bulb had the markings Great Value GVL2T81227 Intertek T8120003 2700K. It appears to be an LED tube style bulb that replaces the traditional T8 Fluorescent Bulb. The fire was contained to the fixture itself which was located in the ceiling.

No one hurt Made Nov 26, 2019, sent to the company Dec 20, 2019, published by CPSC Jan 7, 2020
The product, in the report: Great Value brand (Walmart) 24inch Manufacturer: INTERTEK Zhenjang Qianging Electronics LMTD Product identifiers: 5005914 GVL2T81227 20W/‘24T12 Linear Fluorescent Bulb
About 60 days ago I purchased, from Walmart, their brand of 24 inch LED light bulb as a replacement for a burned out fluorescent bulb. This bulb had the Great Value stamp on the base and shows the manufacturer as INTERTEK. Below the Intertek logo was the number 5005914. Also found on the bulb was the number GVL 2T81227.
On the morning of November 25, 2019, upon flipping the wall switch to start the bulb, there was a loud POP and the smell of electronics burning. I removed the light cover to find that the bulb had exploded at one end and several shards of glass were found in the cover. There was also black smoke on the inside of the fixture, adjacent to the area of explosion, indicating a fire had been present. This is a ceiling light with two bulbs. There was no damage to the other bulb, which was a regular fluorescent bulb, and it continues to function properly.
Other writing on the bulb base was: Zhenjang Qeanging Electronic Co. LMTD 20W/24T12 Linear Fluorescent Bulb
Photos were taken.

No one hurt Made Feb 9, 2019, sent to the company Feb 25, 2019, published by CPSC Mar 12, 2019
The product, in the report: LED replacement for use with ballast 48 inch.
The product is an led replacement tube that went up on fire or smoked. I have tried dealing with Walmart (GREAT VALUE BRANDED) but they keep sending surveys, but not an response. The lights at first caused one ballast to fail. I decided to try them in a second troffer, and they were not consistently lighting. By chance I removed one bulb and they started lighting more regularly. Until they didn’t. One of the 2 bulb in the troffer went up on fire it appears. It scorched the troffer, the glass for the tube was cracked all along it’s length, and especially at the ends. The stink is high. I have pictures. I am not recieving an answer or response from Walmart.. So now maybe the CPC will care more.
I have pictures, and still have the piece of the tube..

The product, in the report: Walmart - Great Value Brand - 4 BR30 Floodlight, Energy Saver Halogen Bulbs, 40 Watt -65 watt equivalent, Life 2000 hours
This form quit on me unexpectedly in the process of filling it out, not sure if you have a partial of this report already- therefore Redoing this form, Walmart Great Value BR30 Floodlight, 40 watt, energy Saver Halogen light bulb - installed in my kitchen light recently. The outside glass part of the light bulb just popped and exploded all over the kitchen and eating area with great force, shards of glass everywhere, this is dangerous, as normally my daughter and I would be sitting underneath that very light bulb, eating and doing homework. That light bulb was installed directly over the edge of the kitchen island and chairs where my daughter normally sits.
The halogen light part is still working and in the light socket but there is no glass housing. It is unclear how I will be able to uninstall that light fixture as it was installed with a suction cup-pole as the ceilings are high.
The pop-explosion is dangerous. We could have been injured by the exploding glass -it was very forceful and glass shards were strewn across 2 rooms.

The product, in the report: Great Value LED Daylight Deco Light Bulb, 40W equivalent, Walmart #: 551270792
Approximately one month ago, I replaced the three 60 watt traditional candelabra light bulbs in my front porch light fixture (a wall sconce affixed to the brick front of my townhouse) with three Great Value brand LED light bulbs from Walmart. The bulbs are sold in Walmart Stores as “Great Value LED Daylight Deco Light Bulb, 40W equivalent”, Walmart #: 551270792.
When I came downstairs this morning, I noticed that my porch light was not on, which was odd because it is on a timer that turns them on at dusk and off at dawn, and it was still dark outside.
I opened the front door to inspect the fixture and discovered that one of the bulbs appeared to have caught fire, as it was completely blackened and charred, with the housing cracked from heat. In addition, the center post was charred and the entire fixture smelled like burnt electrical wiring. The fixture is now completely inoperable, even with new bulbs, and I have had to purchase, and must install a new wall sconce outdoor light fixture. I feel fortunate that it did not cause a house fire and was contained within the light fixture.

The product, in the report: Great Value Flourescent Bulb 23 watts
Consumer stated that his spiral fluorescent light bulb, inserted in a lamp burned up inside of the ceiling fixture in his kitchen. The consumer was getting ready to leave his home when he heard a loud popping noise from the kitchen. The consumer had came back to the kitchen to find the bulb had popped and the light bulb was hanging from the wires exposed. The consumer’s brother in law shut off the power to the fixture and replaced with a common filament light bulb. The consumer is unsure if his bulb is similar to spiral bulbs that have been recalled and is not sure if it has not been recalled.
The bulb has cinge marks and blackening at the base. The bulb emitted a burning smell of a fire and did not actually see any flames from the light bulb. The consumer decided not to call the fire department at the time of the incident. The bulb is 23 watts, 330AMPS and 120 volts. The light bulb had not flickered just before the incident. The consumer mentioned that he had the bulb in the fixture for 4 months and the bulb was on for 8-10 hours.
The consumer has yet to call the retailer . The consumer called the firm yesterday and stated that they would call him back and they did not. The consumer called the firm today to inform them of what happened and was told to submit the bulb in a package back to the firm. The consumer intends to send the bulb back after he takes photographs of the bulb. The damage of the product did not spread to the structure of her home. The light bulb did not burn the light fixture in which in the bulb was inserted. The consumer was not harmed by the product.
