The product, in the report: GE Soft White 100w (72w rated) Halogen A19 modified spectrum energy efficient medium base general purpose unit.
I keep a light bulb on continuously year round in a very small Heat Pump blower cabinet room in our home to help combat mold and mildew in that room. I check it regularly and noticed the light was off to find that the bulb had a catastrophic failure. The bulb is a GE Soft White 100w (72w rated) Halogen A19 modified spectrum energy efficient medium base general purpose unit. It had exploded and glass shards were all about on the floor. What caused this failure I can not say specifically but the internal FKR 72w4 H8Q (Halogen) light source failed by blowing a 1/4” hole in the element thus causing the outer bulb shell component to explode.
This is unacceptable and potentially highly dangerous from exploding glass parts, internal unknown gas laden dust and the threat of a fire hazard.

No one hurt Made Dec 1, 2015, sent to the company Dec 9, 2015, published by CPSC Dec 31, 2015
The product, in the report: A GE 75W incandescent modified spectrum replacement light bulb in a single receptacle wall fixture. Purpose to provide light in a bedroom.
A GE light incandescent light bulb exploded into hundreds of tiny and large pieces several minutes after it had been turned off. I turned off the wall fixture light in the evening of 11/14/15. Several minutes later I heard a fairly loud ‘Pop’, got out of bed to look for the cause. Fragments of light bulb were scattered onto the floor, bed covers, and nearby dresser. I found fragments 10 feet from the fixture.
I gathered the larger fragments onto the dresser where I took digital photos. I have attached a digital photo to this report. The tiny fragments required multiple and extensive vacuuming to remove from bed covers and carpet.
