Made Oct 13, 2016, sent to the company Nov 3, 2016, published by CPSC Nov 18, 2016

The product, in the report: 3-way halogen light bulb 50/100/150 watt

(09/06/2016) The consumer turned on the floor lamp that had the 3-way light bulb using the low 50 watts. After the lamp had been on for about 10 minutes, he heard a loud (booming/bomb) sound. He stated after hearing the noise, he went to check his home. He found debris (little pieces of the glass bulb) all over the bedroom floor. He unplugged the floor lamp from the electrical outlet.

There was no damage to the lamp. The consumer did not try another bulb in the lamp because the base of the 3-way light bulb was stuck in the light socket of the floor lamp.

(9/13/2016) The consumer contacted the manufacturer and spoke with [REDACTED]. He advised the consumer to mail the light bulb to the company. The consumer refused and stated he will send the product to FTC to investigate.

The consumer stated that the manufacturer sent him a coupon for $12.50.
