The product, in the report: 75 Watt T4 G9/Bi-pin Dimmable 2900K Halogen Bulb (Set of 5) By Bulbrite Industries sold along with: Brunon 4 - Light Dimmable Bath Bar By Willa Arlo Interiors Bulb Included: No Quantity: 1 Quantity: 1
I purchased a bathroom light from Wayfair. Next to the light on their site are 75 watt halogen light bulbs that they recommend for use with the product. When the product arrived, the directions for the light say to use a maximum of 60 watt bulbs. There are four bulbs to the light fixture, so the bulbs suggested and sold by Wayfair for this particular light far exceed the safety limit. The heat generated by halogen bulbs is intense, so this is an obvious fire hazard. Luckily we caught it before installation. I brought it to Wayfair’s attention that the bulbs need to be taken off the website NOW, but they are telling me it will be reviewed in 3-4 business days and see what goes from there.

The product, in the report: Bulbrite Halogen 150 watt bulb.
Several months ago I replaced 2 Bulbrite 150 watt halgoen bulbs Q150CL/EDT, item #614151. One burned out fairly quickly. Today the other exploded sending shards of red hot glass around the room. The glass ruined a sofa and the wall to wall carpeting by burning it. luckily no one was hit by the glass shards.

The product, in the report: Bulbrite Halogen BR-40 Flood light 100W 120V Item #695100 UPC 739698695105
A lightbulb exploded showering the office with shards of glass. The bulb was purchased from Topbulb.com and manufactured in China by Bulbrite located in Moonacie, NJ. The lightbulb is item #695100 and the UPC is 739698695105. It is a halogen BR-40 Flood 100W/120V. There is no marking on the packaging or product that it is listed with Underwriters Laboratories or ETL for safety. The office has 3 other identical bulbs installed and no other bulbs were affected or the circuit breaker tripped. I contacted both Topbulb.com and Bulbrite. Topbulb.com did not want to do anything because they only sell the product and have never heard of this happening before and blamed it on an electrical problem. Bulbrite didn’t want to take responsibility either. After stressing that I was concerned for the safety of the product and that I had the bulbs installed in other parts of the office they finally said to send an email with the details of the incident and a copy of the receipt of purchase. It appears they are only going to address the monetary issues and not the safety issues.

No one hurt Made Aug 23, 2011, sent to the company Sep 19, 2011, published by CPSC Oct 3, 2011
The product, in the report: Energy Wiser Compact Fluorescent Bulbs by Bulbrite
Gentlemen: Attached are photographs taken this morning regarding what I believe to be defective CF15G30SW bulbs manufactured by Bulbrite. They include a picture of the light fixture (8 lamp....I also have a similar 4-lamp fixture). Both light fixtures were new and installed in March, 2010 as part of bathroom renovations. The fixtures that they replaced were very similar. I have been using these bulbs for several years since changing over the entire house to CFL’s in April, 2009. I had been concerned that these bulbs were burning out way earlier than their anticipated 8000 hours. Last year I started marking the installed date of each replacement bulb on the bulb itself. The first one to go was Lamp #1 which had been installed on 4/27/2011 and had burned out on approximately 6/25/2011. I say approximately in that I may wait 1-4 days to change one out. I noticed that the outer glass shell had cracked and I attributed this (at that time) to a defective shell. Lamp #2 was installed on 3/20/2011. It had burned out (turned dark) on approximately 8/17/2011. On 8/20/2011 I briefly had smelled something burning and went through the house trying to find out the source of the smell. On Sunday, 8/21/2011 I replaced that bulb and one other (which did not have an installed date on it). I immediately found the source of the burning smell as soon as I had unscrewed the bulb. It appears that it arced and had burned through the surrounding plastic like material creating a small rectangular shaped hole and some smelly material had leaked out. It also had a cracked outer shell which now tells me that Lamp #1 may have been heading for the same fate as Lamp #2. These bulbs could have easily started a fire or dripped a hot material on someone. There is obviously a problem with materials, assembly or both and the CPSC needs to investigate this issue more thoroughly. I have both of these lamps in my possession along with 12 in fixtures and 6 new ones in boxes. I have been purchasing these bulbs from “www. lightbulbsusa.net”. Thank you for your prompt attention to this serious matter.
