The product, in the report: 6 foot type 1 aluminum Werner ladder 366 Single sided Type 1 duty rating Purchase from Lowes

Defective Warner 6 foot aluminum ladder. While Taking down gazebo, (on an even concrete slab) he was standing on 4th step taking screws out of the gazebo the ladder was straight in front of him. The ladder sheared at second step from the bottom (left side only). The ladder dropped from underneath and husband fell backwards( from 4th step) and landed on his back. We went to the hospital and had internal bleeding and two fractured ribs as a result of the ladder. Had to be transferred from a smaller hospital to one that dealt with trauma. Where he spent two nights in the hospital. The ladder was bought from Lowes less than a year ago. Was garage kept ,barely used and had a 250 pound weight limit. At the hospital, they weighed my husband with all his clothes on and shoes at 240lbs .

The product, in the report: Werner 8’ aluminum step ladder

I am Facilities supervisor at a local Entertainment Venue/Restaurant. My assistant and I were installing extra insulation in the kitchen ceiling. We were working on a flat level floor with plenty of room to wok. I was on a 6’ ladder and my assistant was on an 8’ ladder. Both ladders were standing firmly on the floor and set up safely. My assistant was standing on the 5th rung from the floor on a Werner Model # 368 Mk21 8’ aluminum ladder and the left main leg folded inward causing the ladder to collapse to left and to the floor. He fell on the ladder back first but was unhurt except for some minor bruising and he was sore the next day. The ladder was in perfect condition - always kept indoors in a clean, dry storage area. It is rated for 250 lbs. He weighs 190 - 200 lbs. Even though he was ok, I had him fill out an accident report with our H.R. lady. I have photo’s of the ladder right after it happened and also of the acoustical ceiling that came down with him.

I am a builder and had a new Werner 6006 step ladder collapse on me causing a fall. I was using the ladder to inspect electrical wire installation on a new home construction. The fall cause me to land on my side causing a shoulder and hip injury. The ladder has a soft plastic top and flimsy locking braces. The suspect ladder twisted a little which causes the braces to bend and topple over. The ladder braces were fully locked before climbing and I am within the rated weight of 250lb.

The product, in the report: This was a 17ft werner multi tasking ladder

My partner and I were setting up a werner multi purpose ladder to paint a wall approximately 8 to 10 ft high. The ladder was set in the step ladder position. She climbed up a few steps and felt the ladder seemed a little different than earlier uses that day so she came back down and made sure everything was locked into place. The second attempt, she slowly climbed back up reassured that everything was in place, and around the fifth or sixth step the ladder broke apart in 3 pieces across the floor. She suffered a large laceration to the head, ankle and shoulder sprain and was covered in paint along with the clients flooring and furniture. She was rushed to the nearest hospital where she recieved staples to the head, cat scan, and xrays of her foot and shoulder.

The product, in the report: Werner Type II Medium Duty/Commercial use non-nonconductive fiberglass 6 foot stepladder; FS206, load capacity 225 pounds.

Yesterday morning (15 Aug 2017) I was standing on our Werner 6 foot Type II Medium Duty/Commercial use non-conductive Fiberglass stepladder. When suddenly it collapsed under me in a split second throwing me off to the right. Both cross braces had bowed with vertical cracks in the fiberglass appearing where the cross braces are attached to the ladder. The skin on the back portion of my right elbow was shaved off apparently by the bowed out cross brace on the right side of the steps as I fell to the ground. I also hit my head on a lawn border brick. I was taken to the local emergency room by ambulance where my complex lacerations were sewed up. I have always been safety conscience and ensured the ladder was fully deployed and on level ground. I weighed 231 pounds yesterday which is 6 pounds over the specified weight limit of 225. I believe I was standing on the third step from the ground and in the process of moving to the fourth step.I do have pictures of the ladder following its collapse, but am having trouble uploading them to your site due to the special Flash Player requirement. This morning I spoke with an individual in the construction business regarding my accident yesterday. He informed me the construction company he was with threw out this stepladder as soon as it reached five years old. Apparently, they were aware that the fiberglass in this ladder tends to lose its strength and fatigue over time resulting in this type of accident. My ladder was probably about eighteen to nineteen years old. The individual looked at my damaged ladder and said the same thing had happened to him where the cross-braces suddenly buckle outward, and their attachment points in the fiberglass exhibit vertically running cracks.

The product, in the report: Step Ladder: Werner Saf T Master, Model 365; pin 465774. Weight limit 225 lbs.

Ladder legs collapsed while I was standing on it causing me to fall, hitting my head. My neck/shoulders are sore but no head injury. Leg and knee is. Weight limit of 225 pounds was not exceeded, and I was not on the top step, per the safety advise noted on the ladders label. My weight is about 147 and I had one piece of paper in my hand at the time that I wanted to store in a container on the loft in my outdoor shed. The ladder was open all the way hinges secure, with the feet of the ladder on level ground. I have a photo of the ladder and of the bruises on my legs, but am unable to upload them as your pages goes grey when I press “add file”. The ladder is a Werner Saf T Master, Model 365; Pin # 465774. We have owned it for approximately 10 years, so it is not a brand new purchase, but has been stored inside a shed, so is not rusted.

The product, in the report: Werner blue fiberglass 8’ step ladder

December 2014, I was taking items out of the rafters in the garage. The 8’ Werner Fiberglass 250lb ladder I was standing on tipped - I grabbed on to the beam and hung for a few seconds until my 25 year old son helped me down. March 2015, the same thing happened. This time I was alone and could not grip the beam and came crashing down. I was hospitalized for a week with multiple leg fractures including a Tibial Plateau Fracture. I was in a wheelchair for about 4 months and am still recovering.

I am 45 years old and have been using ladders my entire life. In my garage, I use to use a 6’ ladder and thought the 8’ ladder would be safer. The ladder is only a few years old. When I bought my ladder, I also bought one for my father who is 30 years older and constantly uses ladders. He really didn’t like the ladder feeling it was not stable and only used his once. My dad felt that the hinged cross bars were too high and flimsy. Because the ladder tipped twice within a short period of time seemed rather unusual (I had only used the ladder twice in the mentioned time frame).

Basically, I am writing because I do not want someone else to get hurt. I am fortunate I only suffered a broken leg.

The product, in the report: product no T6203 by the Werner ladder company. This is a 3 foot trellis ladder

I am a wallpaper hanger. For 45 years, in using various 3 and 4 foot stepladders, the occasion arises where one must jump clear of the ladder in a tip over incident. In using the Werner ladder T-6203, I faced the need to get clear of the ladder. This ladder collapses in an unsafe manner. All things break, and they should be engineered to break safely. This ladder is so designed to collapse the opposing side in a failure. This is dangerous. A ladder can break on one side, and yet give enough stability for the user to save them self. This ladder does not. To my misfortune this defect caused a broken arm, and 3 months of lost income.

03/16/15 My husband was caulking a house while standing on the 3rd rung from the top of a Werner FS108 8’ fiberglass step ladder when all of a sudden the ladder collapsed and fell over causing my husband to break his leg in five places. He is still in a wheel chair and has had two surgeries since this incident. The ladder’s right hinge completely gave out and bent in a 90 degree manner. The ladder is rated for 250 lbs and my husband only weighs 195 and the only tool in his hand was a caulk gun.

The product, in the report: Werner 6’ aluminum stepladder, model 366

Hello, I purchased a Werner 6’ aluminum stepladder from The Home Depot on 11/12/14 I used it for the 2nd time on 02/26/15 and less than 30 minutes later I was on the floor. I was on the 4th rung removing items from one side of my vaulted ceiling. I was going to have my living room painted soon. I was almost done and placed my right foot on the 3rd rung to descend and the next thing I knew was that my body was falling and being banged up on the wall and on the ladder. The ladder was flush with the wall avoiding any need for reaching. I was on the floor, dazed, nauseas and the back of my head ached as well as my entire body. I was afraid to move so after about an hour I looked over at the ladder and noticed that the leg buckled-it was bent inwards and the bottom rung middle section was lifted as well. I filed a claim with Werner and in the end they stated that my ladder isn’t defective and that I just had an accident. They offered to replace the ladder and sent a release form but I declined. I don’t want anything to do with these people especially since the ladder is only 4 months old now and they refuse to accept that their products can and do fail. They’ve gotten away with too much and for too long now and someone has to stop them. I’m disabled, cervical spine issues and I had a few operations in 2003 and 2004 however I have use of my arms and legs thank God but the only healthy cervical vertebrae that I have left is the C1 and i’m concerned that it herniated as well since the E.R. doctor said that I strained it. I also hurt my shoulders, arms and legs and had many contusions. I had 2 cat scans-brain/head and neck. I also had elbow xrays and the P.A. gave me an rx for pain medication. I also saw my MD after the incident. I informed everyone as to what occurred-exactly and truly. I’m new in Florida-5 months tomorrow but I was able to contact several attorneys but they couldn’t assist. My head still hurts-bumped it and my neck, arms and shoulder still ache as well. I’ve read too many stories about my ladder as well as with their other models-it’s truly sad and quite unfair.

*I believe the hinges are faulty, also once you touch the ladder your hands turn black.

    Thank you.

    * I tried to add my photos here but for some reason your program isn’t accepting-maybe it’s a glitch?

The product, in the report: eight feet (8) step ladder Model #FS108 P/N 100514-123 REV F rated 275 pounds load

I purchased an 8 feet step ladder from Home Depot in October of 2013 Werner Model FS108 P/N 100514-123 REV F, this is a fiberglass framed with aluminum rungs and supports. On February 8, 2014 while trimming a tree, the ladder clasped. I fell 6.5 feet and suffered severely bruised ribs and shoulder injury on the right side. The ladder was on flat ground. Looking at the ladder, the fiberglass C channel support had separated at the second rung, on the right side. The delamination caused the ladder to fail. Total weight was 208 lbs. Ladder rating is 275 lbs. A quick calculation utilizing the actual thickness and dimensions of the ladder indicated this ladder is under designed for the intended purpose. I contacted Werner and Home Depot concerning the incident. Home depot has said they have no responsibility and Werner should be contacted for all items. Werner has responded with no solution or offers. This ladder is a very popular item and appears to be under designed for any off load. This is extremely dangerous and I believe should be tested for strength. My correspondence has been with [REDACTED].

The product, in the report: Werner 6’ fiberglass stepladder rated for 250 pounds.

On January 26th, 2013, I was at my new house painting up inside one of the skylight tunnels in the dinning room. My wife was right next to me so that she could hand me paint filled rollers and what ever else I might need while working in the confined space.

I was using a 6’ Werner FS106 fiberglass ladder, rated for 250 lbs, that was maybe a year old and is always stored in my shop. I was on the fourth rung from the bottom. Due to the confined area I was working in, there was no way possible to be leaning hardly at all in any direction, so I was centered over the ladder.

In a nut shell, all of a sudden the ladder dropped out from under me and I fell out of the skylight tunnel and landed on my right scapula on the hardwood floor. I ended up in the emergency room with a small pneumothorax injury and spent two days in the critical care center.

I went over to the house yesterday to look at everything. Nothing has been moved since the accident and I am going over today to get some photos. The right side rail was bent in directly below the bottom rung about 45 degrees towards the center of the ladder. The lateral support for the bottom rung looks like it pushed up the bottom rung a good 3” in the middle. None of the fiberglass parts are torn looking and I can not see any threads from fiberglass tearing. Both side webs of the C channel side rail are mushroomed outwards a lot. On the left hand side rail, you can see a similar, but small, amount of deformation.

The product, in the report: Werner 6006 fiberglass steplader

Werner 6006 fiberglass stepladder failure The morning of Friday July 12th 2013 was the beginning of a beautiful summer day. The temperature was somewhere in the upper seventies with virtually no wind. So I decided to trim the burning bushes on the south side of my house. I got out my Werner 6006 stepladder, set it up fully extended and on level ground in front of one of my bushes. With my electric trimmer in hand, I climbed several feet up the ladder and began trimming. I was almost done when I felt the ladder lean to my right. I responded by leaning to my left. However, the ladder continued to lean more and more to the right. The ladder went out from under me and I fell about 4 feet to the ground. I landed on my back and to the left of the ladder. I laid on the ground for several minutes as I worked to regain my normal breathing pattern. Once I felt able to do so, I made my way into the house and set on our couch. My wife came in and asked me what was wrong. See told me a looked white and very uncomfortable. I went to my doctor early the following week wear he sent me to the lab to be X-rayed. The results indicate that I have a fracture in my lower back. I closely inspected the ladder. What appears to have happened is that the right leg failed just above the bottom step. This failure allowed the right leg to buckle inward which caused the bottom step to buckle upward in the middle. The buckling of the bottom step pulled on the left leg causing it to buckle inward. This should have never happened. The ladder looked in good condition when I set it up and I never expected this kind of failure. The two main legs on this ladder are channelized fiberglass with the face being about 4 inches wide and the perpendicular being about 1 inch wide. I believe the fiberglass 1 inch perpendicular portions of the legs were not made strong enough to withstand the lateral forces that may be applied to it from normal use. I weighed about 200 pounds at the time of this accident and the ladder is rated at 250.

    To whom it may concern:

On July 13 2012 I was painting on your ladder at the [REDACTED], in Lake Ariel Pa. 18436. This ladder was a new ladder bought specifically for painting. I was on the second to the top step when the ladder twisted somehow and went out from underneath me. I fell backwards about 8 to 9 feet in the air and landed on the concrete floor. I injured the whole right side of my body. I knocked the wind out of myself and couldn’t breathe for a while. When I got my wind back the pain was unbelievable. My right arm swelled up like a balloon, there was pain in my ribs and lower back. The ambulance took me to the hospital because they thought I had a broken arm. The x rays showed I didn’t have any broken bones which was a miracle. What I did have was a lot of pain. I couldn’t bend my arm for two or three days to eat, that’s how bad this was. I still can’t use my arm fully, and to this day I still have a lump on it.

I believe this ladder is a bad design and should be fixed before somebody gets hurt really bad I was lucky, if you want to call it that that I didn’t break my back or get killed falling that far on to a concrete floor. Your ladder has metal braces on every step except under the bottom step which is the most important to stabilize the ladder and it has a plastic brace. I believe this is what caused the ladder to twist. This ladder, like I said, was on a concrete floor which was perfectly level. I am enclosing pictures of your ladder showing how it cracked at the bottom and along the seam. This is a fiber glass ladder and should have never done this. The bottom aluminum step is also split.

The product, in the report: 6’ Stepladder

The ladder I was using collapsed and I fell to the concrete floor below. Werner model 366 6’ stepladder. I was on the top step but the ladder was very stable on a flat concrete floor. I reached for a flashlight which was up and to the side which I expect shifted my weight (205Lbs.) to one side and a little bit angled instead of straight down and the ladder simple gave out. No tip or slip or anything other than structural failure. While using the top step is not within the usage guidlines, this sore of use is not out of the ordinary and I am certain many consumers would use it just as I was from time to time.

The product, in the report: 12 foot step ladder marketed by Werner Ladder (Greenville PA)

I purchased a 12foot step ladder from Home Depot online. The ladder was manufactured by Werner Ladder. The ladder arrived here missing two rivets in the right rear spreader brace ( I later found an extremely mangled rivet on the floor of my garage, I am convinced the damage to the rivet could not have occured in the successfully installed position). Fortunately for me (and all concerned) the “defect” was obvious. Had this omission occured in a structurally significant portion of the ladder (not readily noticeable) it could have resulted in significant injury or even death. The missing rivets were the same as used in the rest of the assembly. I have asked both Home Depot and Werner Ladder for corrective action, such action visible enough to me to allow me to exit this matter. Thus far neither party has given me an acceptable reply. I have my e-mail notices and replies if you need them and can assure me that I am legally permitrted to share them.
