  1. No one hurt

Made May 12, 2016, sent to the company Jun 1, 2016, published by CPSC Jun 22, 2016

The product, in the report: It is Medline and has a ref # MDS86321WP lot 3R140782327 and is made in China It is for safe ingress and egress from a bath tub which mounts on the side of the tub as a safety grab bar

This product by MedLine has a design flaw which makes it very dangerous. It should be recalled. The screw which tightens the product to the bathtub has a knob for titghen that is not permanently affixed to the screw. The knob is what holds the apparatus together. So when you tighten the apparatus to be firm enough. it is pushing the knob off the screw and soon the whole apparatus falls apart.
