The product, in the report: Kohler Archer Drain Cover
I have a Kohler Archer shower pan in my shower. I fell about a month ago due to the cover lifting up after I stepped on. I contacted Kohler and they sent me a replacement which is actually worse than the first one and therefore I just fell again and hit my head on the tile. There are numerous reports on [REDACTED] about people getting injured from them.

No incident, no one hurt Made Dec 24, 2023, sent to the company Jan 3, 2024, published by CPSC Jan 26, 2024
The product, in the report: Bathroom, metal, wall mounted, shower handle. Three photos attached.
No injury, I am reporting the possibility of injury to a person’s head or shoulder as a result of bending over and contacting a Kohler shower handle when standing backup. The shower handle protrudes from the wall (horizontally) almost four inches and the vertical handle itself is four inches which tapers to a point. A few days ago, a home contractor finished installing a shower in my home. The contractor selected the shower handle, manufacturer, and style. An injury could occur should a person standing in the shower, and while the shower handle is in the “off” position, drop an object which comes to rest near the shower wall, a person bending over to pick up the object, upon straightening back up the person could strike his/her head or shoulder against the bottom of the tapered shower handle. Another example, should a person be cleaning or wiping down the shower stall and while cleaning close to the wall where the handle protrudes, the person could raise up and strike a head or shoulder against the bottom of the tapered shower handle. After conducting different scenarios, I believe the back of the head striking the bottom of the tapered shower handle could cause of the most serious injury.

First aid Made Mar 7, 2022, sent to the company Mar 10, 2022, published by CPSC Mar 24, 2022
The product, in the report: Kohler Freestanding Air Bath
I had a Kohler Freestanding Air Bath tub installed on January 20, 2022. Since then my daughter and I have recieved burns on our skin from the tub. After filling the tub, getting in the tub, and then turning on the air jets, the initial blast of water out of the air jets was so hot that we were burnt. It happened first to my daughter and I thought she did something wrong. However, when I took a bath the same thing happened to me. We now both have burns on our hip that match the pattern of the air jets. I tried to contact the company but their phone systems says they are too busy to take calls, and the website did not have a help section for this type of issue.

Emergency room Made Aug 31, 2015, sent to the company Sep 9, 2015, published by CPSC Sep 23, 2015
The product, in the report: Acrylic shower base with center drain cover.
We have an Kohler acrylic shower base, Archer model (K-9397-**). The center shower drain is covered by an acrylic plate, roughly 8 inched by 10 inches (part number 1062356**). The cover plate is held down by two small suction cups. The cover plate must be periodically removed for cleaning the drain and over time the suction cups can break free from the cover plate. The best demonstration of this can be seen on the You Tube video EzQd9wC2vT4.
A week ago my wife stepped on the cover plate while exiting the shower. The plate came loose and her foot went into the drain hole, causing her to fall. Fortunately the glass shower door was open and she did not fall through it. She did however suffer multiple bruises and an injured knee that required a trip to the emergency room.
I reported this event to Kohler. I suspect that they know that the cover plate needs more robust suction cups because they replaced the plate at no charge even though it was out of warranty. This is a disaster waiting to happen
