The reports, as CPSC published them
Reports people made to the U.S. Consumer Product Safety Commission. CPSC publishes each one after sending it to the company named, and doesn’t guarantee they’re accurate. A report isn’t a recall.
Each report is the words of the person who made it, as CPSC published them. The company’s comments, where CPSC published any, are set apart as the company’s own.

The product, in the report: Delta In2ition 5-Spray showerhead with handheld combo, chrome finish, approximately 6.63-inch round showerhead. The product combines a fixed outer showerhead with a removable handheld center portion connected by a flexible hose
I purchased a Delta In2ition 5-Spray showerhead with handheld combo in Chrome from Lowes on December 30, 2025.
Beginning approximately one month ago, the center handheld portion of the showerhead began unexpectedly detaching from the main showerhead while the shower was in normal use. This occurred multiple times.
On one occasion, the handheld portion fell out and struck me in the head. Fortunately, I was facing away from the showerhead and was not injured.
On August 25, 2026, the handheld portion detached again during use, fell to the shower floor, and broke.
I am reporting this because the showerhead was less than eight months old when the failure occurred, and I believe the repeated detachment of a relatively heavy component during normal use presents a potential injury hazard. Had the piece struck my face or eye, the result could have been more serious.
I have photographs of the broken fixture and can provide proof of purchase from Lowes if needed.

Delta 16 in. x 1 in. Multi-Grip Tub Safety Bar in White (removed) Per the Home Depot website Model # DF585 UPC Code # 885785437483
As you can see in picture Bar 02.jpg the rubber pads on the legs of the bar slide down When it is tightened which results in a loss of grip on the sides of the tub. In turn while using This to exit the tub my wife fell backwards when the bar released and fell back into the tub. Fortunately, she was only bruised with no permanent injuries. I immediately removed this Bar and found one that was much more secure when attached to the tub. [REDACTED] [REDACTED] In looking at the failure mode the glue was not sufficient to lock the rubber pads to the bar, I Was considering swapping out the rubber pads with [REDACTED] double sided automotive tape, but Did not do this. I also considered finding some rubber pads that had a nipple on them and Drilling holes in the side plates to insert the nipples that would keep them from sliding, but Searching rubber pads with nipples did not provide my expected search results.

The product, in the report: Delta Shower Door Delta Lyndall 36 in. x 64-3/4 in. Semi-Frameless Contemporary Pivot Shower Door in Chrome with Clear Glass Model # 2406349
A Delta designed shower door uses a plastic hinge on the bottom of the door to hold the door on. This plastic piece deformed in normal use after 2.5 years, causing the door to fall off and almost causing injury to my wife. Luckily she was holding the door by the handle and was able to hold it from striking the tile floor and shattering. I was able to remove the door from my wife and safely place it where it would not shatter.
