  1. Seen by a doctor or nurse

Made Jan 25, 2016, sent to the company Feb 2, 2016, published by CPSC Feb 18, 2016

The product, in the report: Visions 56-60x72-inch Frameless Sliding Shower Door

Product: (DreamLine Visions 56-60x72-inch Frameless Sliding Shower Door - I do not see a serial number but on the remaining glass is stamped: Tempered 16CFR 1201-II ANSI Z97.1-2004 1/4UA SGCC 3960) was purchased July 6, 2014 from Overstock.com and installed mid August, 2014. At 7:45am January 25, 2016 my husband stepped into the shower, closed the doors (the enclosure has two stationary panels on each side and two sliding panels that meet in the middle). After closing, the panel on the right side shattered and imploded on him. He had not turned the water on yet. He experienced some cuts (arm, thighs, abdomen, chest, feet) (none requiring suturing) and some glass in his right eye that came out with flushing (I am a nurse practitioner with 15 years of emergency department experience and I cared for him immediately). I took photos with him in place (in case information was required about the pattern in which the glass fell) and then gave him shoes to enable him to get out to safety. I removed glass from the anterior surface of his thighs, feet, arms, neck, face, and torso. He then showered in another bathroom and I swept up the glass into the corner of the shower to avoid further danger to us or our pets.
