Made Aug 13, 2014, sent to the company Sep 3, 2014, published by CPSC Sep 17, 2014

The product, in the report: Glass shower door shattered after midnight last night. My son and I were awoken by a very loud crash thinking a large tree limb had landed on the house but found the bathroom covered in glass. No one was injured but both of us were very shook up over the incident. The shower was just installed around the 1st of April. There was no noticeable cracks or any problems with the functionality of the shower door. Some damage was sustained to the flex wall in the shower as well as the new flooring from the glass. The serial number off the box was #90000002289988 Olympia Halo Shower Door (by MAXX) 47”X 78 3/4”

I had a MAXX shower installed during my recent bathroom remodel April 1,2014. The shower contains a flex wall on 2 sides and glass/glass door on the other 2 sides. No noticeable defects in the glass were noted and no problems with the shower door of any king since the install. (ex. sticking, coming off the track etc.) My son and I were awoke after midnight last night by a very loud crash. We thought a large tree limb had fallen on the house, but found the glass shower door had shattered and glass was everywhere. Fortunately no one was injured.
