Made May 15, 2014, sent to the company May 23, 2014, published by CPSC Jun 9, 2014

The product, in the report: Standard tempered shower glass door. It was installed as part of a new home purchase from Toll Brothers in 2011.

On Friday, May 9th 2014 at approximately 1:00 am our shower glass door shattered. No one was using the shower at the time. The entire glass door exploded into countless small pieces. It is a tempered glass door. I did some research online and found that this can happen when there is a defect in the glass. I contacted the home builder (Toll Brothers). Toll Brothers then contacted their installer, but they decided that since the home was build 3 years ago...this was out of warranty and they would not cover incident.

The installer is Frank’s Reliable Glass and their phone number is: 909-594-9252

The manufacturer is Glass Werks and their phone number is: 323-789-7800
