No one hurt Made Sep 8, 2026, sent to the company Sep 14, 2026, published by CPSC Sep 28, 2026
The product, in the report: Frameless sliding shower door with two sliding tempered glass panels mounted in a top track, used to enclose a walk-in shower in a residential bathroom. Delta Everly series, Contemporary style, model SD3226588. Nickel finish hardware with clear glass. Sold as a 60 in. nominal size, 71 in. high, adjustable to fit openings 50.125 to 58.5 in. wide. Each glass panel is 6 mm tempered glass, approximately 29.75 in. wide by 67.75 in. high. Purchased new from The Home Depot on June 20, 2020, order number [REDACTED], Home Depot internet number 302918742. Installed in 2020. The manufacturer sells the replacement glass panel set for this door under part number SDGS060-CL-R. The outer panel is the one that shattered. The inner panel remains installed and has been taken out of service.
The outer sliding glass panel of a Delta Everly 60 in. x 71 in. frameless sliding shower door spontaneously shattered in a bathroom in my home. The door was purchased new on June 20, 2020 and installed that year. It had been in normal daily household use for approximately six years with no prior incident. The failure occurred at approximately 1 PM. I was in the adjoining bedroom and heard the panel break from there.
At the time of the failure the bathroom was unoccupied and the shower was not in use. No one touched the door. No object struck it. It was not being cleaned, adjusted, opened, or closed. The panel broke on its own.
The panel is 6 mm tempered glass. When it failed it disintegrated into a large volume of fragments that covered the shower pan and the bathroom floor and traveled well outside the enclosure, reaching every corner of the bathroom. Cleanup required a shop vacuum, and small fragments continued to turn up afterward. Glass dust covered everything and required careful cleaning.
No one was injured, only because the bathroom happened to be empty at that moment. The bathroom is used daily by 2 adults. Anyone standing in the shower, stepping into it, or walking through the room barefoot when the panel failed would have been exposed to a large quantity of falling and scattered glass with clear laceration potential, including risk of eye injury from flying fragments.
The second glass panel from the same door remains installed. In written correspondence about this incident, the manufacturer stated that the remaining panel could be compromised due to the glass breakage and advised me to use extreme caution when handling it. In later correspondence the manufacturer stated that the remaining panel may have been compromised because required safety components were, in its view, not installed correctly. I have taken the enclosure out of service and the shower is not being used.
I reported the incident to the manufacturer. The claim was denied and the denial was upheld on further review. The manufacturer attributed the failure to installation error based on its review of photographs taken after the panel had already shattered. No one from the manufacturer inspected the door or the glass. No fracture analysis was performed and no fracture origin has been identified. I asked the manufacturer to identify where the fracture originated and what point of contact it corresponds to. That question has not been answered.
The manufacturer has not offered to inspect or replace the remaining panel that it described as potentially compromised.

Seen by a doctor or nurse Made May 18, 2026, sent to the company May 21, 2026, published by CPSC Jun 5, 2026
The product, in the report: Delta SGCC-6272-6mm-U-A. Tempered Safety Glass ANSI Z97 1-2015. 16 CFR 1201
On Saturday evening May 16, 2026 @ [REDACTED] pm, my 13-year-old son entered his bathroom to take a shower. He turned on the shower water, waited for it to warm up, entered the shower and then reached for the handle of the sliding glass shower door in order to close it. According to my son, as he grabbed the handle, the glass shower door suddenly shattered/exploded without warning.
I immediately heard a very loud crashing sound from kitchen and ran downstairs to the bathroom. When I entered the bathroom, there was shattered tempered glass throughout the shower, bathroom floor, and surrounding area. My son was visibly shaken and had multiple cuts/scratches from the broken glass.
Prior to this incident, the shower door had shown no signs of damage, instability, cracking, sticking, or operational issues. The shower door had been installed new in approximately 2017 and had been in normal household use since installation.
The pattern of breakage appeared consistent with a full tempered-glass failure, with the glass breaking into thousands of small fragments. No objects struck the glass, and there was no horseplay or unusual force involved. The only contact described was normal use of the shower door handle.
After the incident, I researched similar occurrences and located reports/complaints involving similar Delta shower doors allegedly shattering unexpectedly during ordinary use.
Photos were taken immediately following the incident documenting:
* The shattered glass throughout the bathroom * The remaining shower panel/frame * The injuries sustained by my son
This incident created a significant safety hazard and caused physical injury and emotional distress to my child.

First aid Made Apr 3, 2026, sent to the company Apr 8, 2026, published by CPSC Apr 22, 2026
The product, in the report: Frameless frosted pattern Sliding Shower Door Glass Panels in Tranquility
I am submitting this report to document a dangerous incident involving a Delta sliding glass shower door that suddenly exploded without warning in my home.
At about 11:00 PM EST, my baby and I were asleep when we were startled by a loud explosion coming from the bathroom. When I checked, I found that the Delta sliding glass shower door had shattered on its own. Glass shards were thrown everywhere across the bathroom and nearby area. This created a very dangerous situation and could have caused serious injury.
The explosion was loud enough to terrify my baby, who woke up screaming. No one was touching or using the shower door at the time. There was no impact or misuse that would explain the glass breaking. It appeared to shatter on its own.
This product poses a serious safety hazard, especially in homes with children. A shower door should not suddenly explode and scatter sharp tempered glass throughout the home. I am concerned this may be related to a manufacturing defect because there have been other reported incidents involving tempered glass shower doors shattering without warning.
I am reporting this incident so it can be investigated and to help prevent this from happening to other families. I am also requesting that Delta address this dangerous issue and provide a replacement and proper remedy.
Product - Delta sliding glass shower door Hazard - Shower door suddenly shattered and scattered glass shards Incident location - Bathroom in home Time of incident - About 11:00 PM EST Injuries - Emotional distress and risk of serious physical injury. Child was terrified by the explosion. Requested action - Investigation, safety review, manufacturer accountability, and replacement of the shower door.

The product, in the report: Delta Commix 54-3/8 to 60 in. W x 76 in. H Frameless Exposed Roller Sliding Shower Door in Matte Black, 5/16 in. Clear Glass SDESR60-MB-R Internet # 316768794 Model # SDESR60-MB-R Store SKU # 1006342063
Delta SDGS060-CLN-R Shower Door exploded as i was in the shower as a result of a nickel sulfide inclusion, an estimated 125,000 PSI of pressure forcefully impacted my body, causing significant injuries and emotional trauma. The blast sent shards of glass in every direction. The glass struck the top of my head, and one of the metal rollers hit me on the forehead with extreme force, causing a laceration. My face and body were hit with shards, leaving me with multiple cuts and heightened sensitivity. The sound and force were so intense that it felt identical to a bomb or explosion inside our home, which is exactly what I believed had happened in that moment. As the glass fell, large shards and fragments covered the floor of the shower. Several pieces embedded in my feet, preventing me from safely stepping out.

The product, in the report: Delta shower doors.
The consumer was at home with her partner cooking when they heard a noise. They started checking the house, inside and outside, but they could not find anything. Two hours later, the caller went to the bathroom, and she realized that the shower glass door had shut and there was glass all over the floor. They didn’t know what could have happened since none was using the shower at that moment. She contacted the company, and they told her they could send her a replacement for that door. However, the caller wanted the CPSC to know to make people aware that incidents like this can happen with this glass door.

The product, in the report: Delta Pivoting Shower Door
Our bathroom had a Delta Pivoting Glass shower door. We have lived in the house for 9 months and the door was installed by the prior owner. There was no indication of an improper installation. In the middle of the night, the glass door shattered to many pieces, sending glass shards all over the bathroom. Luckily nobody was in the bathroom. This is a serious safety concern, knowing that the glass door could shatter and send glass shards flying across the room. with no explanation.

I am writing to express my extreme dissatisfaction and concern regarding this recent incident On May, 12th 2024, the glass shattered unexpectedly everywhere, posing a significant safety hazard risk and causing inconvenience and distress.
Model # SDGS060-CL-R Store SKU # 1000003640 - 29-3/4 in. x 67-3/4 in. x 1/4 in. (6mm) Frameless Sliding Shower Door Glass Panels in Clear.
The glass on one of the doors spontaneously randomly exploded shattered all over the bathroom into numerous very sharp fragments, in the middle of the night What a nightmare...heard a loud EXPLOSION noise, this is was terrifying Listening to to the sound of explosion shattering glass. What is particularly disheartening is that this shower door glass was explicitly advertised and Sold by Delta as Tempered glass, implying its durability and safety by ANSI Z97.1 American National Standard for Safety Glazing Materials and by IAPMO IGC 154-2019 Shower and Tub/Shower Enclosures safety codes and American Standards, however, the fact that it shattered and exploded so dramatically and unexpectedly clearly indicates a severe discrepancy between the advertised quality of the product and its actual performance. This incident has not only caused inconvenience but also raised serious concerns about the reliability and safety of your products by the American safety Standards. As a customer, I expected transparency, honesty, and accountability from Delta company, none of which seem to have been upheld in this instance, they said 5 years warranty and don’t want to honor it, they don’t want to help cleaning up the shattered glass, they don’t want to help install a new glass replacement with better American quality standards. No one hurt Made Dec 5, 2023, sent to the company Dec 8, 2023, published by CPSC Dec 22, 2023
The product, in the report: Delta Frameless Sliding Traditional shower doors.
On Sunday, November 5, 2023, while we were having breakfast, we heard a large crash. What we discovered was one of the glass shower doors purchased and installed in 2020 by a professional contractor had totally “disintegrated”. Glass was everywhere. There was no apparent reason for this to happen. We never noticed any chips, scratches or defects in the glass. The shower is only used by two people, we are the only residents. Had someone been in the bathroom at the time, they would have been seriously injured, not to mention had they actually been in the shower. The explosion of glass damaged the frame along the bottom. (small dents) The tracks were filled with glass shards and fine glass powder that had to be removed. Glass had blown across the bathroom, under the sink and under shelves. The contractor purchased the materials, which included the shower doors, from Home Depot and the doors have “Delta” labels on them. The doors are supposed to be warranted for 5 years. Apparently, Delta uses Liberty Hardware to supply the doors to Home Depot. After some difficulty contacting Liberty, we were able to make contact and supply the information they required to make a warranty claim on November 9. The claim was denied on November 10th citing that a T-strap on the opposite side had been installed incorrectly using an extra screw. On November 13, we wrote back to Liberty explaining that while the particular T-strap in question indeed had an extra screw, it was of no consequence since the exploding door was installed on the opposite side and did not contact the screw in question. This was evidenced by the photographs showing the majority of glass on the floor and in the track on the opposite side of the shower. We asked that the matter be escalated to management. On November 30th after no answer, we sent a final request for management intervention. On December 1, the Technical Provider replied that the matter was forwarded to a manager. As of now there has been no response. Based upon other similar complaints regarding Delta Shower doors listed on your site, I believe Liberty is supplying a faulty product and hiding behind supposed installation errors. Again, someone could have been seriously injured by the shards produced when the glass literally exploded had the shower actually been in use at the time. Photographs are available.

The product, in the report: This is a 60 x 58 1/8th inch bathtub sliding door. It was installed by a renovation company and checked after installation by the project manager.
While holding a glass panel, part of a 60 inch bathtub door newly installed, the glass shattered while attempting to set it back into its track. This happened on two panels. No pressure or undue force was applied to the doors. I was holding one by its side edges when it shattered and had just finished placing the other into its track when it shattered. The glass was tempered and I sustained only multiple superficial cuts. The door is manufactured by Delta (Liberty Hardware Manufacturing Corp., Winston Salem, N.C.}. The model is the Delta Simplicity Sliding Bathtub Door with Tranquility Glass, Model 2435527. The item was installed in mid-February 2022. I notified the company by phone after the first incident (see below) and the representative said that I was holding the door in a manner that put too much pressure on the glass. I sustained multiple lacerations (not deep) both times. In both cases, there was no excessive force placed on the doors. In the first instance (March 30th), the door was re-seated in its frame when it shattered, and in the second instance (April 1) I was holding it by the edges getting ready to raise it to seat the rollers into the top frame.

The product, in the report: Delta Frameless half height shower door that is installed on the lip of a tub/shower
Our 8 y.o. daughter was taking a bath when a “Delta Shower” frameless glass door that is installed above the edge of the tub suddenly exploded, and shattered into thousands of pieces, after she placed her hand on the door. She was showered with glass, screamed, and was covered in blood. She has over 50 cuts over most of her body including her neck, shoulder, chest, stomach, arms, legs, and feet/toes. She required care in the emergency room, for debridement of the glass and stitches for more serious cuts. She had multiple sutures on her left thigh for a wound that went through the skin into the fat of her leg. She also had sutures on her left shoulder from a puncture wound. Our daughter is very scared and traumatized emotionally. One shower door remains in place, but the bathroom and tub are completely covered in glass, some pieces being very small and others are very large pieces. This is not safe for a child simply bathing in a tub without any misuse.

The product, in the report: Delta shower glass doors. They are typical size, about 3’ x 5’.
This happened twice to us. Both Delta shower doors we purchased shattered without warning.
Letter 1:
My wife and I were installing Delta shower doors yesterday. We got to the point where we were ready to put the doors in and everything was going fine. The glass door is very heavy but I was able to handle it. I brought it into the bathroom and my wife was standing next to me. As I began to raise it up to put it into place the entire door shattered into a thousand pieces. Photos are attached. My wife and I both got cuts, but it was nothing serious thankfully. We are okay.
I did not drop the door. It did not hit the side of anything as I was lifting it. It literally seemed to explode in my hands. We don’t know what caused this to happen since it did not hit anything in the bathroom and I had a good grip on it at the time. Why it broke is a complete mystery to us. I could have understood if I would have dropped it on the marble floor, but I had it in my hands when it broke.
We have been using Delta shower doors in hour shower now for over 15 years. We have bumped against them countless times and never had one break and/or shatter as this one did. Have you ever seen this happen before? We would like to know what the warranty is on this door. Is this door under warranty and can you send us another one? Our order is attached as well. I look forward to hearing from you.
Letter 2:
[REDACTED] Attachments Thu, Oct 14, 2021, 2:04 PM to DeltaShowerDoors <customersolutions@deltashowerdoors.com,
I don’t know what is going on with the manufacturing process of your glass shower doors but you definitely have a quality control issue. I was lifting up the door to install one of them a few months ago and it exploded in my hands my wife and I were both cut severely. The other day I was simply sliding the door to close it and the other one exploded. I was standing about two feet back so fortunately I was not cut too badly this time. The photos are attached. It took us three hours to clean up the mess since the little tiny shards of glass cover every inch of the bathroom. You sent me out a replacement for the first one that exploded and that is the one that explored. We want a full refund at this time, not another replacement. We are done with your exploding glass shower doors. We are lodging a complaint with the Consumer Product Safety Commission.
We are going back two shower curtains. These glass doors are just way too dangerous. I had glass shower doors for 15 years before we replaced them. We banged them many many times and nothing like this happened. Delta shower doors seem to explode over nothing. You have a real problem. You can send us a check or refund our credit card. Either way, after two exploding doors oh, we want our money back, not just replacements.
They refunded our money in full.

During the evening of March 31, 2021, our shower doors made a loud “pop”, then rained down glass shards on my two year old son, and husband. My son was in the tub at the time and sustained cuts to his body and lacerations to his hand that required a trip to the ED, x-rays, and stitches. My husband also sustained lacerations to his arms and hands. I have enclosed photos to show the extent of damage to our bathroom. We are still cleaning up glass shards and my son is still receiving medical care for his injuries as a result of this event.

The product, in the report: 48” clear, dual sliding show doors (mounted from the floor up
The consumer’s 25year old daughter turned on the water and was preparing to get into the shower, when the glass exploded. The glass disintegrated and the daughter was left holding the handle in her hand. She sustained multiple lacerations to her legs, feet, hands & forearm.
The consumer immediately took her daughter to Boulder Community Health Hospital and the ER staff examined her. The medical staff picked glass from the daughter’s skin, cleaned her wounds and glued the larger lacerations. The daughter was released the same day.
12/9/2019: The consumer called the manufacturer and reported the incident to them. Their rep. told her that the consumer could purchase a new door and send in the receipt and they would cover the cost. The rep. also recommended that the door that did not shatter, should be replaced as well (precautionary). The consumer was told that she could file a different claim from the door reimbursement, through their Claims Department, but they were unsure of what the outcome might be.
The consumer’s husband installed the shower door, because he has extensive experience in this area. Additionally, this was one of those do-it- yourself shower doors kits.
The consumer searched online and saw that this problem has been reported by others many times, on different sites.

The product, in the report: Delta sliding shower door -
September 5th, 2019 my son was in the bathtub and suffered two lacerated tendons and ligament in his right hand due to a shattered Delta brand shower door. He was in the tub when one of the two rollers fell off the track above. One corner of the shower door was resting on the side of the tub. When touched the shower door to raise it back up, it shattered completely. Instead of small, pebble-sized glass shards, many pieces did not break and fell in hockey puck sized shards, one of which severely cut his right hand. A visit to the emergency room was required, followed by surgical repair, and weeks of physical therapy. A follow up procedure is likely in order for him to regain complete mobility in his hand.

Attempted to open door by knob and entire door EXPLODED sending the tempered glass flying over the bathroom and slicing my arm and hand, with bleeding. I was dressed which prevented covered body parts from being pierced. Could hear popping sound continually after glass shattered in small pieces on floor and in shower. Glass did not simply break, but EXPLODED all over bathroom !!!! In pain from sliced arm and hand, have pictures.

The product, in the report: Tempered glass shower door
Few minutes after 5am this morning, one panel of my glass shower door “exploded” and shattered into small pieces of sharp glasses and tiny, powder-like sharp glass pieces were flown out of the bathtub/shower and out to the hallway. See attached photo.
No one was touching it, tugging it or even near the door. Delta website warns not to do certain things to avoid the explosive breakage but it was spontaneous without any provocation. Luckily, no one was in the shower at that time but if it occurred in only few minutes later, those sharp glass pieces would have launched onto my skin. The “explosion” set off the burglar alarm. My hands and toes got few minor cuts while trying to clean up all the small glass pieces all over the bathroom. It is nearly impossible to safely clean all small glass pieces and even after sweeping the floor with wet paper towel for 10 times, there are still tiny glass pieces everywhere. I am attaching a photo of [REDACTED] surface where you can see the tiny pieces of glasses after 10th sweep (after wet paper towels) - Please, zoom in to see the glass pieces!
This is a dangerous product but after some research, it appears that there maybe a safety film or glazing solution that may prevent the shards of sharp glass pieces from flying out to cause injury. I feel that all manufacturers of temper glass shower doors should be required to incorporate a similar safety measure to prevent injuries.

The product, in the report: The shower door was just replaced by the home owner about 3 weeks back. The contractor was a single man shop and he bought this shower door from Home Depot. The name of the company who manufactured the shower door is called Delta. They mostly sell from Home Depot.
This incident occurred on June 28th 2019. My son was taking a shower and suddenly, I heard a loud explosion. I also heard my son screaming. I ran to the bathroom and saw that the shower slide door glass was shattered into million pieces all around him in the tub. He was badly injured and had to be stitched at various places to heal from that accident.

My husband was exiting the shower and as he closed the door behind him, the glass wall broke and shattered. Although it is “safety glass” there were thousands of glass shards/crystals all over the bathroom and he sustained several cuts on his feet and body. There was no indication that the door was loose on the hinge, that the hinge was defective, nor was there a crack in the glass door. We are still unsure how it happened in the first place.

Hello,
I recently had my glass door explode while I was in the shower. There were no cracks or any issues with installation that could’ve caused this issue. I was in the shower and it sounded like a bomb went off. The glass pieces fell around me and cut my legs and my feet. I had to walk out of the shower with glass everywhere which led to additional cuts and bleeding. The issue is still unresolved at this time.

I have a 4 year old and a 6 year old. Four year old inside the shower, six year old outside shower. They were pushing on the door from each side, it broke. Had sharp edges that caused multiple areas of cuts. Both had to go to the ER at 11:30 at night. Very traumatic for the children.
The glass should not have had sharp edges. My 4 year old had a long piece pierce his foot and the 6 year old has a bad cut on her finger.

The product, in the report: Delta Portman 60 in. x 70 in. Semi-Frameless Sliding Shower Door in Chrome with Clear Glass, Model #SD2832937 Home Depot Internet #300499032
I purchased the “Delta Portman 60 in. x 70 in. Semi-Frameless Sliding Shower Door in Chrome with Clear Glass” from Home Depot (Model # SD2832937 Internet #300499032) on Apr 20, 2018 and a contractor proceeded to install it on April 26. When handling one of the panels, it shattered suddenly with no contact except with the contractor’s hands (i.e. he did not bump it against anything). There were no injuries. He purchased a spare panel from Home Depot and finished the installation. I took several showers using the door with no issue. Then my condo manager was addressing an unrelated issue on May 2, 2018 in the bathroom, and the other (original) panel shattered when he opened the shower door. There were no injuries this time either. Both panels that came with the door have now inexplicably shattered, so I believe this to be a manufacturing defect, and one that poses an unreasonable and substantial risk of laceration. Luckily, both incidents happened with fully-clothed individuals present, but that easily might not have been the case since it’s a shower door.

Sliding Bathtub Door Glass Panels in Clear (1-Pair) Completely transparent glass panels provide clear visibility Combine with Delta track and handle for a complete shower door Spot Guard glass coating resists hard water stains
Delta Bathtub Shower Door Model # SDGT060-CL-R , consists of 2 clear sliding glass panel doors within an aluminum frame. Glass panels slide using small wheel assembly at the top of the panel that fits into the top frame, as well as the bottom of the glass panel is guided by a small plastic guide installed on the bottom frame. The frame and doors were professionally and correctly installed according to the instructions provided with the unit, leveled, etc. However one of the glass panels (the one that is used the most) consistently comes off the track, causing the person using it to have to replace the glass panel door wheels back on the track at the top of the frame.
This time, while attempting to replace the panel back on the frame, the glass panel “exploded” into a zillion pieces, causing personal injury from glass cuts on hands and arms, accompanied by a fair amount of fright and shock (emotional, not electrical). This happened without the glass panel being subjected to any undo stress, impact or other “reason” that it should have disintegrated as it did.
While I am able to purchase a replacement door, they are only sold in pairs - not convenient or right. Besides that fact, I am unsure as to whether these tempered glass panels are safe!

The product, in the report: Delta Frameless Shower Doors 60# ( 2 doors)
On November 15th at approximately 6:30 a.m. I was beginning to take my shower when the frame less glass shower door I was closing suddenly exploded. The doors were tempered glass and the manufacturer is Delta. I purchased and installed these doors almost a year ago from Home Depot. I followed all instructions included with these doors. I’m skilled and installing and doing handy man type of work so I know I installed them properly.
This door exploded loudly , without any warning at all. I was pushing on the towel bar and the door was not shut when this happened. The force of the explosion sent glass everywhere and knocked the towel bar out of my hand or blew my hand away. Nonetheless and stunned, wet and naked, I wasn’t sure what had happened. My feet were covered in glass and my right hand was bleeding. I stood still to gather myself and to see if I had sustained any other injuries. Considering that glass was covering my feet and glass was everywhere, I slowly pulled one foot out at a time. I didn’t noticed any injuries to the top of my feet, I had to step out onto glass to get out of the shower and in so doing sustained a minor cut on my right heel. Once out I reached for a towel and put my feet on it as I shuffled out of the bathroom into the hallway to find a towel to stop the bleeding on my hand. I found one in my laundry room and wrapped around my bleeding hand. Once, I did that I saw that I had received 2 gashes in the top of my right hand. I quickly found some hydrogen peroxide to pour into the wounds to clean out and to deal with any bacteria. I then took some pictures. Once I was composed and bandaged the sight, I found a gash along my right forearm as well. I cleaned it as well. I then got dressed and headed to my primary Physician, Dr. [REDACTED] in Tomball where I live. Dr. [REDACTED] determined that I could probably do w/o sutures and so his assistant, applied the type of surgical tape that pulls wounds together to help them heal. She also treated with some type of topical solution. I also had to have a tetanus shot and then sent on my way.
Once back home I began to do some research into these type of doors and was stunned to learn that these doors due to their manufacturing process have the explosive power of 10,000 PSI and that the breakage is so fast that it exceeds 14 times the speed of sound. In other words, they are ticking time bombs. I will attach a you tube video of an expert witness in Chicago that elaborates on this subject matter. I’m also attaching pics of my hand and the glass breakage. I have not filed any claims with my insurance company, nor have I contacted Delta or Home Depot. I thought it better to obtain legal advice and direction first.
The wounds have healed up nicely. However this could have been much worse and any part of that glass could have hit a major artery and could have resulted in more serious injury or death. Imagine, a small child like my grand children taking a shower and this happening with glass coming down on them. That situation has occurred to others if you search the internet.

It instantly exploded, before it was totally closed, into thousands of pieces, sending glass both into the shower and into the bathroom. I received several cuts on my hands and feet. One cut on my right index finger was large and required a visit to the emergency room where I received 6 stitches to close it. The door had worked fine up to that point and had not been damaged in any way.

On March 7th, 2016 the shower door in the master bedroom of our duplex exploded while our tenant was preparing to bathe his 7 month old. The glass shot all across the bathroom and into the bedroom, narrowly missing the infant. The tenant had many small cuts as he was closer to the bathroom at the time. This is the second time this has happened with the same product. The first time was about 6 months prior. The incident happened to a different tenant in the other bathroom within the same apartment. At the time, I thought it was an exaggeration. You see...he also described the door as exploding.

The product, in the report: Frameless Sliding Glass Shower Door
My sliding shower glass door literally exploded into thousands of pieces. It cut my hands in several places. The door was functioning fine. I simply grabbed the door handle to slide it closed when the door exploded. Glass shards went everywhere including the ceiling and adjacent room.

The product, in the report: Frameless Glass Shower Enclosure Model # is 422061
I had just gotten into the shower and closed the shower door behind me when I heard a pin drop. Then CRASH!! The right shower wall completely shattered and fell all around me along with the shower door (which was still intact). Yes, I know exactly what caused it. The pin fell off the shower door hinge and the weight of the door could not be held, so the right side of the glass wall completely shattered. I had to walk out of my shower and bathroom over shards of glass (barefoot of course) and all the while shaking from the shock of what had just happened.

The product, in the report: Bathroom Shower Door
Caller ordered glass shower door December 3rd, 2014, it was delivered about one to two weeks later and had it installed by contractor who was remodeling her bathroom. She had no problems with the shower door until May 29th, 2015 when her house guest stepped out of the shower after she turned the water off. Once the house guest stepped out of the shower, the door shattered with one of the pieces of glass cutting her on her right hand. The caller went to the retailer, spoke with [REDACTED] who checked with is supervisor then returned this callers phone and provided her with the manufacturer’s contact information.. She spoke with the manufacturer June 8th, 2015 and was advised that they would replace her shower door. She is reporting this incident because she doesn’t want just the shower door replaced, for safety sake she is asking for an entirely new replacement of frame, doors etc be provided at manufacturer expense with the manufacturer installing the product. The manufacturer has advised her that the frame would only be replaced if the door shattering ruined the frame. He informed her that these doors do not shatter on their own.
The house guest treated her cut and did not require medical attention

I purchased a delta simplicity 59 3/8”x 58 1/8” sliding by pass tub door in poilshed chrome with frameless clear glass. Price paid around 200 dollar, transportation $35, installation cost $150. after two weeks of use, the door went slightly out of track due to the light weight of the product, it was easy to go off track, I was outside the tub and i just pushed slightly on the door to raise it to get on track, it explode all over my body and on my hair scalp and on the floor like a slush of ice poured from a machine. I did not put even lots of pressure on it, i was naked ready to get in the shower. being mature person 50 years old, i did not move and i rushed to Kaiser hospital then they can clear up the galss fragment off my hair scalp. they did not rub my head rather they vaccum it and then pour water gently for 15 minutes till all fragments are cleared. I need this produce to be pulled off the market and evaluated for strength and material data sheets need to be evaluated, I also need compensation for all the damages i have incurred.

The product, in the report: Delta 60” sliding glass bathtub doors, model “Rain”.
I had finished installation of bathtub enclosure glass door structure with glass doors installed and operating. I needed to remove the doors in order to make an adjustment. I removed the outer door, was holding it in both hands, and it just completely exploded in my arms. I didn’t drop it, I didn’t bend it or hit it on anything. It just shattered completely. Very shocking!
