The product, in the report: Wen 4750 generator

The fuel line on my Wen 4750 generator sprung a leak while it was running. I went out to turn it off and there was a puddle of fuel on the ground and the line was squirting fuel. This is a fire/explosion hazard waiting to happen.

The product, in the report: Wen 3500-watt portable generator with 4 gallon gas tank; product code: 56352.

The decedent was performing renovations to his trailer residence; the trailer had no electricity and mainly utilized power via an extension cord to a neighboring residence. Reportedly, a second source of power was needed and he borrowed a portable gas-powered generator. The decedent was last known responsive at approximately 1930 hours on 12/05/16 via text message and was discovered unresponsive, fully clothed, in a seated position on his couch in front of the television. Multiple electrical items were in the “on” positions, but were not working; the items were plugged into an extension cord running to a “WEN” 3500-watt portable generator with a 4 gallon tank which was empty; the generator was not running at the time of scene examination, but the engine switch was flipped to the “on” position, the fuel valve was set to the “on” position, and the voltage selector was flipped to the “120v” position. There appeared to be no obvious form of ventilation throughout the residence and upon discovered, all doors and windows were closed except one window which was slightly ajar. A carbon monoxide test returned results of 8 parts per million approximately 2 hours after discovery (with the door being left open).
