No one hurt Made Jan 8, 2018, sent to the company Apr 9, 2018, published by CPSC Apr 23, 2018
The product, in the report: LectroFan ASM1007 white model. Please note that I had previously heard about the recall: https://www.cpsc.gov/Recalls/2017/power-adapters-sold-with-lectrofan-sound-machines-recalled-by-asti-recall-alert but had checked and my unit is not on the serial # list covered by that recall. But the AC power adapter does have the same model # as that one.
My daughters (4 & 5) were planning in their room in which the Lectrofan sound generator was plugged into the wall. The oldest came to tell us that something was wrong. I came down and the power brick had mostly broken off from the outlet but the piece with the electrical prongs was left in the outlet, sticking out, with the backside of the metal prongs completely exposed. I believe one of them hit her leg against the power brick while playing and that dislodged the plastic pieces of the adapter, which appear to have been held together with only a little glue. It is only by the grace of God that they came to tell me rather than touching it.
