The reports, as CPSC published them
Reports people made to the U.S. Consumer Product Safety Commission. CPSC publishes each one after sending it to the company named, and doesn’t guarantee they’re accurate. A report isn’t a recall.
Each report is the words of the person who made it, as CPSC published them. The company’s comments, where CPSC published any, are set apart as the company’s own.

Hurt, care not known Made Sep 3, 2018, sent to the company Sep 11, 2018, published by CPSC Oct 2, 2018
The product, in the report: Direct TV ( AT&T) Receiver/DVR Box
As I immediately reported to DirectTV and AT&T, our receiver/DVR box took on fire after hours of melting right in the bedroom where we were sound asleep in the middle of the night of January 15th.. Would we not have been woken from suffocating fumes of melting plastic, we could’ve been killed right in our sleep. It was a horrible experience. My husband jumped out of bed first not knowing what was going on. He then discovered that the fumes were coming from the box where small flames already started showing from the side vents of the receiver. He grabbed it and burned his fingers at first, then grabbed a towel, disconnected the box and carried it into the back yard. We couldn’t stay in the house that night as the smell was unbearable, so we took our 3 dogs and went to a friend’s house, leaving the windows open.. Direct TV picked up the box for further ‘investigation”. We went on cleaning up the smell, that costed us thousands of dollars. Our mattress had to be replaces, carpets shampooed several times, our five thousand-dollars TV started malfunctioning and so on, and so on. I placed a claim for damages, but have not heard from them for months. I’ve requested the final investigative report on the unit many times, to no response. Any liability of AT&T and Direct TV was denied. I wrote to the office of the president warning of the danger of these defective unites, to no response. When their technician came to pick up the unit the next day, he examined it and stated that it overheated because the valve that was supposed to turn it off failed to do so. I am in outrage and disbelief how irresponsible and neglectful this behavior by Direct TV is.

The product, in the report: combination satellite receiver / digital video recorder
While watching TV using my DirectTV system, the Receiver/DVR stopped working and began to emit smoke.
I was not home at the time, my girlfriend had the presence of mind to quickly disconnect it and put it outside, she mildly burned the side of her hand when she touched the top of the cabinet (while attempting to quickly remove the connected cables)

The product, in the report: DirecTV receiver box, model #HR23-700
On September 14, 2011, I turned on my television and the DirecTV box, the television came on, but, the DirecTV box did not. Followed instructions given by DirecTV to check the box, it still did not work. They are sending a new box.
On September 15, 2011, in the morning was walking into the bedroom when I noticed smoke and the room smelled like an “electrican burn” was in progress. Noticed smoke comming from within the DirecTV box. unpluged and took it outside. Smoke was very powerful and toxic. Aired the room with fan and open windows.
Have box. Holding it for your inspection. Will not return it to DirecTV unless told to do so.
PLEASE ADVISE WHAT YOU RECOMMEND...
Thank you.
