The product, in the report: CVS PowerXcel USB Wall Charger 1.0, Teal, SKU 973625

We bought two “PowerXcel USB Wall Chargers” from CVS Pharmacy in November 2014 and have used them to charge various devices around the house (primarily [mp3 player] [REDACTED]es, [REDACTED]s, etc.). My son had his [mp3 player] [REDACTED] plugged in and went to remove the charging cable from the device when he heard an arcing sound coming from the charger body and saw an orange glow coming from the middle of the charger. When my son saw what was happening, he unplugged his device, leaving the charger connected to the wall outlet through an extension cord. My wife came and unplugged the extension cord from the wall. The charger body is partially melted and has blackened arc residue on the openings for the USB port and the 120V prongs. The SKU for the product on the CVS website is 973625.
