The product, in the report: Oculus quest 2 controller
While in standard use, a Meta Quest 2 right-hand controller suffered a catastrophic, spontaneous internal electrical failure. The device rapidly overheated, causing the internal circuitry and plastic housing to melt, and began actively emitting toxic electrical smoke. The user immediately extracted the standard removable AA battery to cut power, but the device continued to out-gas chemical smoke and melt internally due to the severity of the short circuit. No physical injuries occurred, though the thermal event created an immediate fire and respiratory hazard in the home. When the manufacturer (Meta Store Support) was contacted regarding the hardware safety failure, they refused to honor the product warranty or issue a replacement unless the consumer placed the fire-compromised, charred, out-gassing hardware into the commercial mail stream using a standard return label. Despite being informed that the unit retains an intense, toxic, burnt electrical fire odor and out-gasses chemical vaporsviolating USPS Publication 52 Harmful Matter Regulations regarding obnoxious odors and safety hazards to transit personnelMeta explicitly instructed the consumer via email that they were ‘confident the device poses no shipping risk’ based entirely on looking at a digital photograph. When the consumer refused to violate federal transit safety guidelines by mailing an un-inspected, internally melted fire hazard, Meta Store Support unilaterally terminated communication and closed the support ticket without resolving the product safety issue

1/19/2022: The consumer contacted the manufacturer about these concerns. The firm’s automated system had a message stating: “ We are sorry, but we are unable answer your call at this time. We are unable to provide support or govern this matter.
Sometime later, she was able to speak with a rep. (name unknown). The firm’s rep. spoke about copyright information, but the consumer was not calling about copyright concerns. The consumer said that the rep. was unwilling to assist her further.
The consumer’s child asked if the brightness could be turned down on the product, because it was impacting their ability to use the product.
The consumer placed the product on her head and immediately the light inside the product impacted her eyes and caused her to have headaches for days.
She tried to call the BBB, to report this matter, but was unsuccessful. The consumer believes that when one places the product on their head, it attacks the users eyes.

We purchased an Oculus Quest II from target for our son for Christmas. On Christmas morning, he opened the box and began using it for the day. After several hours of use, he developed a bright red/purple swollen rash below each eye in the shape of the headset and had some lighter redness and rashiness on his forehead. Upon a [REDACTED] search, I found out that the foam has previously caused reactions and noticed that there was mention of a silicone cover. Going back to the box, I found the cover in a pouch of paper that was previously unseen. The markings on the box were not clear that the silicone existed or that rash rash was a potential issue. We allowed the rash on my sons face to subside, which took over twenty four hours and first aid and then allowed him to try it for thirty minutes with the silicone cover. Within an hour of taking it off his face, the rash had returned. I reached out to oculus and they provided no solution other than returning it to target, which I did and thankfully target accepted it for a full refund. My son has a medical appointment today and will have the rash looked at. It is now faint but his skin is still raised, bumpy and pink.

After using the oculus quest 2 headset regularly, I had a significant skin reaction on my face where the foam pad made contact with my forehead . I consulted my doctor because of this rash that turned into blisters that took over a month to heal. It was very uncomfortable and I immediately stopped using the oculus headset once this skin reaction appeared. I have permanent scarring above my eyebrow from this event. I decided to report this after receiving a email from the retailer “Amazon” and verifying that my oculus headset’s serial number is a product that has caused several adverse skin reactions.

The foam headset included in the package for the Oculus Quest 2, I bought on Amazon caused my skin to break out after the first week of having it. It was not usable and would cause itchiness and breakouts on my forehead after 10 minutes of using there after.

Oculus Quest VR system, playing [REDACTED] game, led to injury when mouth slammed into console table. Mouth was deeply cut, swollen and bleeding on inside. Skin broken and bleeding on outside. Wounds still healing 1 week later.

We bought an oculus quest VR headset for our son’s 17th birthday and his face has broken out in an alarming rash that has required medical intervention. The company (Facebook) suggests only that they send some kind of cover if we fill out a form! It’s a known injury that occurs in what they say is less than 1 percent of users; but that seems awfully unlikely as it is discussed widely on [REDACTED] etc.
