  1. No one hurt

Made Dec 9, 2013, sent to the company Dec 16, 2013, published by CPSC Dec 31, 2013

The product, in the report: The 2 Wire GateWay is a combination modem, router and WiFi unit for interfacing to the Internet in the home or office. It is usually placed on a desktop or shelf and is continuously powered. It was supplied to me as part of an AT&T Internet service upgrade. The Gateway described herein is actually the second one provided to me by AT&T. It was a replacement for the same type of unit I received (serial number 371314008072) on 1 November 2013 and which failed, in a non-spectacular fashion, after only 10 days of service. This unit was returned to AT&T at their request on 15 November. The latest failed unit was returned to AT&T at their request on 19 November after a third Gateway was received and put into service in my offic.

The AT&T Internet Gateway/modem was received via UPS from their Ft. Worth supplier the afternoon of 13 November 2013 and placed into service by 8PM on that date. It was connected by cord to the supplied 115 VAC to 12VDC, 3 amp power unit. The power unit was plugged into a wall outlet in my home office. The telephone input line and n Ethernet cable was then at attached and the Gateway was registered online with AT&T. The system operated properly & the Ethernet cable was removed as only tne WiFi functton was to be used. On 15 November at approximately 11 AM I was using the Gateway WiFi connection with my [REDACTED] when the [REDACTED] reported the WiFi link was suddenly lost. I looked over at the Gateway, which was located on my desk about three feet from me and noticed all the panel indicator lights were off. I stood up to walk over to the Gateway when I heard a snapping and crackling sound and a large cloud of acrid white smoke emanated from it. There was a strong oder of burning electronics. I picked up the Gateway and saw, through a ventilation grill, small flames and/or sparks inside the plastic case. The case top was quite hot to the touch. I immediately disconnected the power unit from the wall outlet, removed the phone connection, and took the Gateway outside the house. The smoke dissipated and the Gateway cooled off. The Gateway had been powered continuously for only about 39 hours when it failed. I am very concerned that this AT&T provided product could have started a fire, especially since it was near papers on my desk. A major conflagration could have resulted. I’m lucky I was home when the incident occurred.
