The reports, as CPSC published them
Reports people made to the U.S. Consumer Product Safety Commission. CPSC publishes each one after sending it to the company named, and doesn’t guarantee they’re accurate. A report isn’t a recall.
Each report is the words of the person who made it, as CPSC published them. The company’s comments, where CPSC published any, are set apart as the company’s own.

Hurt, no treatment Made Jan 26, 2018, sent to the company Feb 6, 2018, published by CPSC Feb 21, 2018
The product, in the report: Vizio surround sound 5.1 channel stereo speakers with subwoofer. UPC code: 845226015328 Model: SB3651-E6 Part number: 10615000241 Serial number: SZZOWJAT3802276 Unknown code on box: (01)00845226015328(21)001173802276 (I have photos but can’t upload them on your system)
I have these speakers: Vizio SB3651-E6 36” 5.1 Channel SmartCast Soundbar System connected via HDMI cable to the HDMI ARC port of a TCL-branded TV (Model 55P605). When the speakers are left in the “on” position, with the input device turned off (i.e., that’s the TV in the “off” position, which actually leaves it in a sort of “stand-by” mode), there have been at least three separate occasions now (the first being on 01/03/2018 and the last being today, 01/26/2018) that the speakers have given off an EXTREMELY loud, and very high-pitched sound that has lasted from between 10 to 20 seconds. The sound is so loud that it fills the whole house, and it is almost impossible to figure out where it is coming from. The last episode, which occurred today while I was sitting in a room upstairs from the unit, has left my ears ringing (they are still ringing and this happened a few hours ago). The noise is definitely damaging to hearing. Aside and relevant: Through a very brief internet search, I found another person who reports a similar occurrence with this specific Vizio model. Please see the description provided by “[REDACTED]” in Post#[REDACTED] (May 8, 2017) at the following link: http://www.avsforum.com/forum/195-soundbas/2824697-2017-vizio-sound-bar-collection.html It is clear from his post that [REDACTED] has already contacted Vizio about this issue (as I did today, 01/26/2018), yet the model is still on sale. I purchased mine from Costco in November of 2017. Costco is still selling the model. I’ve informed Costco of this issue too.

Hurt, no treatment Made Jan 3, 2018, sent to the company Jan 11, 2018, published by CPSC Jan 29, 2018
The product, in the report: Vizio 38” sound bar system (model SB3851-D0)
Vizio Sound bar (Model SB3851-D0) was in use with [REDACTED] television. Television audio out was sent to the sound bar unit via an optical cable. When going to adjust the volume on the unit itself, an electric discharge between the user and the unit occurred. Arc was severe enough to cause pain in entire right arm and through the left side of jaw, through teeth and into tongue. Sensitivity in jaw and tongue persists 24 hours later (at time of this report).
Ran speakers diagnostic test and speakers continue to function, but the optical channel is completely unresponsive. Ran additional test with auxiliary input (7 mm audio cable), and unit functions, but with persistent audible whine.
Upon contacting manufacturer for investigation of safety hazard, Vizio requests return of the device and offers replacement with new unit. Will not honor a full refund (within 1 year warranty).

No one hurt Made Oct 8, 2014, sent to the company Oct 17, 2014, published by CPSC Oct 31, 2014
The product, in the report: Subwoofer, a piece of the Vizio wireless sound bar system
We purchased a new Vizio Tv and wireless surround system. The tvf and system was turned off and not in use. The subwoofer malfunctioned and caught our home on fire. I do not feel that this product is safe at all.
