Reports people made to the U.S. Consumer Product Safety Commission. CPSC publishes each one after sending it to the company named, and doesn’t guarantee they’re accurate. A report isn’t a recall.

The product, in the report: Infant Optics Wireless Video Baby Monitor (audio + video monitor) Model: DXR8PPZ-A (DXR 8-PRO), [REDACTED]
Infant Optics Video Baby Monitor DXR 8-PRO
The baby monitor intermittently and unpredictably loses audio transmission while the video feed continues to function normally. When the failure occurs, no usable sound is transmitted from the camera to the monitor; at most, faint white noise is present, which closely mimics the sound of a quiet room.
The only way to restore audio is to power-cycle the camera unit. The failure has occurred multiple times over a period of weeks, with no consistent trigger or pattern.
Because the video continues to work, the device appears operational while silently failing to transmit audio. This creates a false sense of security and prevents caregivers from reliably hearing an infant cry or other signs of distress.
Audio monitoring is a core safety function of this product. An intermittent and unpredictable loss of audio materially affects its ability to perform as intended for infant safety monitoring.
On more than one occasion, my daughter was within earshot and crying was heard without use of the monitor while the monitor displayed video correctly but produced no sound.
I contacted the manufacturer. Manufacturer support requested additional troubleshooting that requires waiting for the intermittent failure to occur again in order to observe indicator lights. No resolution or replacement was offered.
The monitor was received as a gift, so proof of purchase is unavailable. Multiple similar complaints describing intermittent audio failure appear in public consumer reviews.
The issue has occurred multiple times over recent months with no visible pattern, most recently yesterday. My daughter was approximately 9 months old at the first occurrence and is now 15 months old.

First aid Made May 11, 2017, sent to the company May 19, 2017, published by CPSC Jun 5, 2017
The product, in the report: Digital Video Baby Monitor with Night Vision
I had purchased the monitor December of 2014, and have used it the past 2 1/2 years daily, without incident. I was sleeping this morning, and the battery in the receiver exploded (the receiver was sitting without being touched on my nightstand, it did not feel hot to the touch the night before nor did it ever show signs of overheating). It landed on my blanket, and caught fire, which I smothered to put out, causing a hole in my bedsheet and a burn on my finger, and multiple burns to my blanket which had to be discarded.

The product, in the report: Infant optics DxR-5 video baby monitor
We were using the Infant Optics DXR-5 baby monitor. The monitor was accidentally dropped approximately 2 feet onto a hardwood floor. When it was picked up the battery exploded. It started spewing smoke and gas before igniting in flames. The battery burned holes in our floor before we were able to safely get it out of the house. It also burned a hole through the trash can we used to get it outside. The floor and walls were covered in black soot and the entire upstairs of our house smelled like toxic fumes. Luckily the nursery door was closed so the baby wasn’t exposed. I sincerely believe under different circumstances this could’ve caused a serious fire or injury.
We contacted Infant Optics about the event. They asked us to return the monitor so they could study the problem but reported they haven’t seen any other incidents like this. They sent us a new monitor, the DXR-8 as recompense. I shared concern that this could happen again and they assured me the device is manufactured using a different material and method and it wasn’t a safety hazard.
