Hurt, no treatment Made Feb 6, 2019, sent to the company Feb 21, 2019, published by CPSC Mar 7, 2019
The product, in the report: 29” Mountain Bike
This Report is in follow up to that of Report # [REDACTED] from 6/16/2017 and Report # [REDACTED] from 9/14/2017 concerning the merchant in question, Pacific Cycle. In response to these complaints, I was contacted by Pacific Cycle in an effort to resolve the defective and used merchandise that had been shipped to me by Wal-Mart on behalf of Pacific Cycle, only to discover -- upon ceasing my patronage of Pacific Cycle after these efforts failed -- that Pacific Cycle is only able to remain operational by foisting used, defective and/or poorly configured merchandise onto consumers through the country’s largest retailers, Wal-Mart, Target & Amazon. Specifically, it turns out that the Mongoose XR Pro bicycle I had purchased on 9/7/2017 from Walmart.com to replace a Mongoose Ledge 3.5 bicycle I had similarly purchased on 5/8/2016 from Walmart.com (that was 5 years old at the time of purchase, as Pacific Cycle itself informed me a little over a year later on 6/2/2017, when the left pedal came off and could not be put back on) was poorly configured to the point where my only viable option at the time was to return it for a refund. When I took the replacement bike (which Pacific Cycle did give me a full refund for) to the only repair shop in town at the time, Bike Tech Macon (www.biketechmacon.com), the day after receiving it, the repair technician on duty was unable to either realign the gears for the bike or correct the damage caused to the bike chain by the misaligned gears that occurred en route to the repair shop. I aligned the gears and straightened out the bike chain as best I could when I returned home (Pacific Cycle offered to pay to have the chain replaced by Bike Tech Macon at the time, which seemed unfeasible to me at the time, given the incompetence shown by Bike Tech Macon, the distance to the store of over four miles and that fact that the bike itself was the only way that I could reach the repair shop -- unless Pacific Cycle were willing to compensate me for transporting the bike by some other means, which Pacific Cycle did not offer to do), only to have the chain come off the bike and break while riding down a steep decline on the way to the local Wal-Mart early in the afternoon of 1/28/2019. Gratefully, the road was strictly a residential one and completely deserted at the time, or my crash could very easily have been a fatal one, although i experienced a serious health setback related to a cardiovascular stent implantation procedure I had undergone on the night of 8/20/2018 (I would have called an ambulance after my crash, if not for the bike).

No one hurt Made Jun 16, 2017, sent to the company Aug 29, 2017, published by CPSC Sep 14, 2017
The product, in the report: 29” Men’s Mountain Bike
On 6/2/2017, the left pedal of the crank assembly on the Mongoose Ledge 3.5 I had purchased from Walmart.com on 5/8/2016 fell off as I approached my apartment complex on the way back from the local Wal-Mart store. Although I followed the label instructions on the pedal and tried to screw it in counter-clockwise with my locking pliers, I soon discovered that the internal threading for the crank arm was almost completely gone, appearing as though it had endured many years of abuse. Upon registering the bike online and initiating an inquiry regarding the issue over the weekend after returning home, Pacific Cycle initially tried to convince me that the damage must be the product of my being sold a refurbished bike by Walmart.com because of its 2012 manufacture date, before finally admitting that used bikes can only be purchased directly from Pacific Cycle and settling on incorrect installation of the pedal as cause for the damage. As I had explained to Pacific Cycle on more than one occasion, however, this could not have been the case, as I had never previously encountered any problems with the pedal over the course of a full year of heavy and regular usage (primarily to and from the local grocery stores -- the nearest of which is a over a mile away -- on at least a monthly basis, but also to my P.O. Box at the Pio Nono Post Office more than 4 miles away). Moreover, I have been very careful to avoid exposing the bike to the elements, keeping it indoors when not in use and not riding it in the rain or through water/puddles. In the absence of prior ownership, any indication that the pedal was installed incorrectly or exposure of the crank assembly to moisture, the damage to the crank assembly threading can only have been caused by the use of inadequate or substandard materials in the construction of it, which should warrant a recall all Pacific Cycle crank assemblies composed of the same materials.

The product, in the report: 24” Mongoose Ledge 2.1 Boys Mountain Bike
We purchased the 24” Mongoose Ledge 2.1 Boys Mountain Bike from Walmart.com. When the pedal is in the forward position, the front wheel can actually hit the pedal! I have pictures to show you. My 11yr old took his Christmas present out for a spin around the neighborhood, and noticed it within the first 200ft...! This is a huge DESIGN FLAW and can potentially cause riders to crash and get injured.

The product, in the report: man’s mountain bicycle
(05/24/2016) The consumer stated that he was riding the bike on an open road. The left pedal was at top stoke when the pedal snapped off. The consumer stated that this was the third time that he rode on the bike.
The consumer was not injured.
(05/24/2016)The consumer contacted the manufacturer to advise them of the incident. Since he did not have the correct information, they were not able to assist him. He got off of the phone to find the information and called back to the firm. He spoke with a different person who mentioned something about a defect but would not repeat the statement.
The consumer was advised that parts could be sent to him. He did not want the parts. The consumer asked to speak to a supervisor. He was sent to someone’s voicemail but he did not get a name. The consumer left a message and someone responded to his call but he was not able to take call.
The consumer called back and left message. He has not received a response.
The consumer added it is not that he doesn’t want the parts, it is that he is not a bicycle repair person and he wants to know that it is safe to ride it and [bike] should have lasted longer than three rides on paved roads. The consumer also added they [mfr] need to send him to a repair shop and have it repaired so that it [bike] is safe to ride.

No one hurt Made Oct 11, 2011, sent to the company Oct 18, 2011, published by CPSC Nov 1, 2011
The product, in the report: Pacific Cycle Mongoose XR-75, 26” mens 21 speed full suspension mountain bike
I am writing to report a defect in a consumer product which has the potential to threaten the safety of consumers using the product.
On October 7th, 2011, I purchased a Pacific Cycle Mongoose XR-75, 26” men’s 21 speed full-suspension mountain bike at Wal-Mart. The model number is R5116WMA, the date code is FS00315AA, and the serial number is SNFSD11C72297.
When I got the bike to my home and attempted to ride it, the bike went less than 500’ before the rear sprocket assembly began spinning freely on the rear wheel hub while pedaling in the forward direction, making it impossible to propel the bike by pedaling.
I removed the rear wheel from the bike, at which time the sprocket assembly fell off in my hand.
I examined the threads of the sprocket and the hub which join the two parts together. I am a marine mechanic, a mechanical design technician trained in geometric dimensioning and tolerancing, and a third-year mechanical engineering student - I am thoroughly competent to examine these threads. Online, I found a specification for these threads as 1.375” X 24TPI. The sprocket is made of steel, with a female thread, and the minor diameter (crest to crest, inside) measures 1.337”. The hub is rolled steel tube, with male threads that measure 1.335” across the crests. The threads on the hub appear to be about 90% intact, the crests had been truncated by contact with the rotating sprocket and appear as 0.005” flats with a 0.035” wide “Vee” trough between.
I believe the threads on the hub were cut approximately 0.040” undersize, which allowed the bike to be assembled with insufficient thread engagement to withstand the stress of its intended load.
The hub is stamped “DACHANG” - this is a Chinese hub made by the company that makes the one shown here: http://www.cndachang.com/ecp.asp?id=31 - I believe (but can not confirm) that the hub shown in the linked page is the same one I have. I believe the recall should extend to all DaChang hubs that are threaded on the same machine that threaded mine - my problem could have been caused by “improper machine setup - tool out of position” (producing a batch of defect parts persisting until the next tool change), or by “improper setup - loose tool” (producing a batch of parts with varying degrees of deviation), or by bad indexing of the machine (producing occasional defect parts for the life of the machine).
I have reported the defective hub to Pacific Cycle, they have offered to replace the entire rear wheel and sprocket set without charge. I will be satisfied, and my own claim with Pacific Cycle will end, when I receive a replacement wheel with the correct threads. At this time, I do not need CPSC assistance with my own claim.
I am reporting this defect in the interest of public safety. The severity of my hub’s undercut, combined with the load imposed by my 200lb body, caused the defect to reveal itself immediately upon the first attempt to use it. But, a less severe deviation from specification, and/or a lighter rider, could allow a similarly defective hub to function for an unpredictable amount of time, with excessively high shear stresses on the hub threads for the entire service life of the bike. In this situation, the undersize threads would be most likely to fail at the exact time the rider needs them most - when he is pushing hardest - to climb a hill or cross a busy street or recover from a momentary loss of balance. A failure - loss of propulsion - at this time could have catastrophic consequences.
I recommend a service inspection recall of all Pacific Cycle bikes built with Dachang hubs, for their dealers (or a set of designated service locations) to disassemble the rear sprocket assembly from the rear wheel hub, and inspect the hub threads for compliance with specifications. I recommend that Pacific Cycle provide a “No-go” thread gauge to each inspection location, where such gauge has female threads cut to the minimum acceptable thread diameter, so that the gauge will thread by hand onto all undersize threads but will not thread by hand onto a hub with acceptable threads.
This recall is urgently needed for all bikes equipped with Dachang rear hubs. It will also promote bike safety overall, by permitting qualified service techs to examine these rear wheels for bent axles, loose, bent or broken spokes, deformed rims, sprocket wear etc.., and to repair any deficiencies they find.

No one hurt Made Jul 15, 2011, published by CPSC Aug 24, 2011
The product, in the report: 2 wheel mountain bicycle
While riding the bicycle on a sidewalk for approximately 25 minutes he noticed that the left pedal wobbly and within a matter of seconds it detached itself from the bicycle and fell off.
He took the bike back into the retailer on the same date and was given another bike (same model number and color) as an exchange.
On 7/14/2011 while riding the bicycle for approximately an hour the left pedal began to wobble. He hopped of the bicycle and the left pedal fell off. He tried to attach the pedal to the bicycle but was not able to do so. He has not contacted the firm and or the retailer but plans to return the bicycle to the retailer.
