  1. No one hurt

Made Jul 24, 2026, sent to the company Jul 29, 2026, published by CPSC Aug 12, 2026

The product, in the report: Each of these Weights has 4 Components. A handle, that is connected to a shackle, that is connected to a swivel and then a heavy round metal ball. ATE 16 Pound Iron Outdoor Throwing Weight. An iron ball that is black. It has a metal swivel, single steel shackle, and steel triangular handle. Bought from 4Throws on 3/16/2026. https://4throws.com --- ATE 25 Pound Iron Outdoor Throwing Weight. Iron ball that is blue. Has a metal swivel, single steel shackle and steel triangular handle. Bought from [REDACTED] --- I have been unable to find serial numbers for either weight. Date of Manufacture is unknown. The SKU is available for the 16lb, but unable to find for the 25lb. Each vendor might be able to provide all this information to you.

I am a thrower with United States Track and Field (USATF). I am also a certified Implement Inspector with USATF. I purchased two new Weights to use during competition. One was a 16-Pound ATE Outdoor Throwing Weight. The other was a 25-Pound ATE Outdoor Throwing Weight. Each of these ATE Weights were bought from a -DIFFERENT Vendor. The weld on each of these weights FAILED. The 25-pound Weight had a catastrophic weld failure during a major track and field competition on 06/27/2026. It happened on the second throw, and it was the weld at the shackle that failed. No one was hurt, but it was a close call. Someone (Official, Athlete, Volunteer or Bystander) could have been seriously injured or killed. A month prior, the 16-pound ATE Weight had a weld failure down at the swivel. This is a very unusual location for a weld to crack and fail. This happened on the first practice session for this weight. Upon closer inspection a very large inclusion could be seen. See picture. There is a circumferential crack that starts and ends at this inclusion. This crack was caught early, before the 16-pound ball separated from the handle. Thus, no injuries. Had I not caught it, serious injury or death may have occurred. The 16-pound failure occurred on 05/02/2026 The person using these two products (implements) is a National Champion Thrower with years of experience. Both weights get inspected before and after each use. They were being thrown onto grass. Both for practice (16-pound) and during the competition (25-pound). Both of these were bought brand new and failed upon first use(s) Also - upon delivery of the 25-pound the weight the handle, shackle and swivel was covered in a slippery silver substance. It covered my hand and made it hard to grip the handle. This was very odd as these components are usually bare steel/metal. I reached out to the vendor who checked his stock of 25-pound weights. They were all covered in this substance too. I was able to clean off this slippery substance with soap water and alcohol. I notified each of the respective vendors immediately after the welds failed on their product. This was too strange to be a coincidence.
