The product, in the report: PHANTOM A10 electric scooter purchased from Costco Item 1819400. Black and gray electric standing scooter with a folding handlebar stem and rechargeable battery. The scooter was used for normal recreational transportation. During normal use the main frame at the steering stem connection fractured and separated completely causing the front portion of the scooter to detach from the deck.

A PHANTOM A10 electric scooter purchased from Costco experienced a sudden structural failure during normal use. A 12 year old child was riding the scooter when the main frame broke into two pieces without warning. The rider was thrown to the ground and suffered facial injuries bruises and abrasions. The scooter had not been modified and was being used as intended. The failure created a serious safety hazard. Photos of the damaged scooter and injuries are available. The product was purchased from Costco and a claim was reported to the manufacturer but the matter remains unresolved.
