The product, in the report: sparkling water machine maker
There is a CO2 cannister that fills bottles of water. She was filling a bottle of water and when she removed the bottle, CO2 continued to leak from the nozzle. She pressed the lever to stop the flow of CO2, but it did not work. Next, she tried to remove the CO2 canister, by unscrewing it. This caused the lever and all of the pieces connected to the lever to explode. Several pieces of the machine flew across the room.
There were no injuries.
She contacted the manufacturer and they asked if anyone was injured. They apologized for the incident. The rep. that she spoke to said that they had no knowledge of prior incidents of this nature.
The firm sent the consumer an identical replacement machine. The rep. advised the consumer that if this were to happen again, to never remove the CO2 cartridge in that instance.

The product, in the report: SodaStream Water Carbonation appliance. Dark Gray and Stainless Steel
SodaStream bottle exploded. This model has a stainless steel bottom and cap. NOT the one tat is on their recall. I was using the unit to carbonate water per the instructions. I pulled the lever a number of times and the unit did NOT Buzz to indicate charging was complete. I then noticed that the bottle was swollen. I attempted to unscrew the bottle to release the pressure when it exploded. My right hand was badly bruised (I have photos) and the sound of the explosion has caused a hearing loss.

No one hurt Made May 26, 2018, sent to the company Jun 4, 2018, published by CPSC Jun 18, 2018
The product, in the report: Sodastream is a machine in which you can fill a bottle with water, attach it to the machine, there is a CO2 cartridge at the back of the machine, you push a button at the top of the machine and it charges the water to make sparkling water.
I was charging the bottle and right at the third buzz the bottle blew the bottom out and it blew the whole machine apart, plastic and parts flew everywhere. I had glasses on which I am very thankful because I could have lost an eye.

The product, in the report: Sodastream water carbonating machine
My Sodastream machine blew up as I was filling a bottle. This happened before the machine buzzed the first time, so I assume that the pressure was not excessive. Water and shards of plastic were blown all over the kitchen. Fortunately I was not injured; I was wearing glasses and a thick bathrobe. The cause may have been the bottom blowing out of the bottle, sending the rest of the bottle up into the filling mechanism.

First aid Made Mar 30, 2015, sent to the company Apr 7, 2015, published by CPSC Apr 28, 2015
The product, in the report: Soda Stream beverage carbonator. I believe that the problem was with the CO2 bottle that is repeatedly recharged.
I have a Soda Stream carbonator and it appears there was a serious malfunction of the valve on the CO2 bottle. This bottle of CO2 really scared me. I put on a new one yesterday and when I finished carbonating it would not stop and I couldn’t pull the bottle forward. I didn’t know what to do so I kept trying to move the bottle back and forth and tapping the button and tapping the bottle (I opened the machine and pulled it out) and all of a sudden it quit so I thought it was ok. Tonight it wouldn’t stop. No matter what I did. I pulled the bottle off and pulled the top off the machine. I tried to unscrew the CO2 and I could tell that if I unscrewed it any more it would shoot off there. The only thing I could do is leave it outside, shooting off the remaining contents, which was full because I had only done one bottle with it. It laid there spewing everything out and the bottle became frozen. After the CO2 bottle thawed out I took it off the head and checked the head and it was working properly. Pretty scary. It appears that the incident also damaged my maker and may not be save to use. It appears to me that these bottles are being reused over and over and may not be getting the proper maintenance. Some of them are pretty old since the machines have been out for several years.

Hurt, care not known Made Jan 6, 2015, sent to the company Jan 14, 2015, published by CPSC Feb 5, 2015
The product, in the report: Soda Stream Penguin water carbonization machine
Was using a soda stream penguin carbonation machine. Have had the machine approx 3 years. Afer the second injection of CO2, the glass carafe exploded into hundreds of pieces. Even though the carafe is enclosed in a structure during the carbonization process, sharp pieces of glass escaped and hit my wife. While she was not hit in the face, that was by luck. There were enough pieces and enough force that she could have been substantially injured.
We reported it to the company last week (via email to their support center) and got no response.

No one hurt Made Feb 10, 2013, sent to the company Feb 19, 2013, published by CPSC Mar 5, 2013
The product, in the report: SODA STREAM MAKER
Soda stream maker blew up on my 11 year old boy while I supervised. The bottom blew out of the soda bottle and water shot everywhere. It made a loud blast and scared us all to death. The plastic piece luckily did not hit anyone. This is very dangerous. We followed directions and this was a newer carbonation tank. The product including the bottle did not expire until 2015 so this was not a factor. We have only owned this since NOv/Dec 2012. We really enjoyed it but I would NEVER let anyone around me use this product again. Very dangerous. I see where grandparents were getting this to share with their grandchildren and I wanted to let people know of the dangers of this product. There needs to be a WARNING: PRODUCT CAN COMBUST UNDERCERTAIN SITUATIONS PLEASE READ! They just say important info .........Very upsetting!

No one hurt Made Oct 20, 2012, sent to the company Oct 31, 2012, published by CPSC Nov 22, 2012
The product, in the report: Red genesis home soda maker with carbonating bottle
My twelve year old son was carbonating some water using his soda stream appliance. He is very familiar with the appliance and the instructions and warnings. He was operating it within the written parameters and the entire appliance exploded. The top of the appliance, including the charging canister rocketed to the ceiling. Water was sprayed all over the room, dripping from ceiling and covering the walls. The explosion shot the appliance with such velocity that he would have been seriously injured had it hit him in the head. It turns out the the bottom of the bottle which came with the device failed. There is a big hole in the bottom of the bottle.
