The product, in the report: Libbey Gibraltar Tumbler Glasses, 16-ounce, drinking glass
Safety report: Product: Libbey Gibraltar Tumbler Glasses, 16-ounce, drinking glass. Manufacturer: Libbey, 300 Madison Avenue, Toledo, Ohio 43604 Incident report: On September 18, 2022 my three year old daughter was holding a Libbey brand drinking glass in her hands. She had lime juice with a few ice cubes, in her glass. My daughter did not drop the glass, and she was holding it normally while watching TV. However, for unknown reasons. the glass broke apart and basically “exploded” to some extent, while my daughter was holding the glass. There were no visible cracks in the glass. The glass was purchased on March 23, 2022 directly from the manufacturer Libbey. The glass has “Duratuff’ on the bottom, indicating it’s “tempered glass,” and thus should be stronger. Again my daughter did not drop the glass. She was, simply holding it. Yet it broke apart in her hands. Photos are enclosed. Child affected: [REDACTED], date of birth [REDACTED], my daughter living with me and my wife and other children.
Sincerely, [REDACTED]

Hospital admission Made Dec 13, 2011, sent to the company Dec 21, 2011, published by CPSC Jan 6, 2012
The product, in the report: This glass is clear and sits approximately 6 inches high. It has a very thick glass base.
While emptying the dishwasher, I reached up to a second shelf to put a glass away. All of a sudden, the glass came apart in two pieces. My reaction was to catch it and, in the process, my right wrist was severely cut. After going to the emergency room and being immediately referred to a hand surgeon, I found out I had severed a tendon in my wrist. I had to have surgery to repair the severed tendon. This occurred in my Arizona home while down there for the winter.
This glass was made by Libbey. It has a very, very thick base. I know several other people with the same glasses. They have reported similar breakages, even having the bottom fall off when ice is dropped into the glass. (Actually, this happened to me a few days before my accident but I was unaware of other breakages at the time so I continued to use the glasses.) We have emails describing instances of breakage that others have experienced. We received these emails after others heard of my accident. I can forward the emails to you if you’d like.
Although I did not purchase it through Amazon.com, I have since found very similar looking Libbey glasses on the Amazon.com website. There are several reviews (1 and 2 stars) that describe sudden breakages.
In addition, my husband visited a restaurant on the east coast and was shocked to have the waiter set down a glass of water in the SAME style glass. My husband inquired about the manufacturer and the restaurant’s experience. The waiter laughed and said “They break all the time. Sometimes we set the glass on the table and it breaks in two pieces and water is all over.” We have this gentleman’s business card (i.e. contact information) if you would like.
Last, but not least, a Libbey glass bowl was recalled last month. It is on your website, along with a picture. It has the SAME thick base as the glass that I am reporting to you.
I still have the broken glass (and a couple unbroken glasses) if you need them to test. These glasses are an EXTREME laceration hazard.
