The product, in the report: Product description: Solo brand disposable plastic forks purchased from Costco.

Product description: Solo brand disposable plastic forks purchased from Costco.

Hazard description: The forks are shedding sharp, toothpick-like plastic fragments during normal use. Pieces broke off into food without excessive force. The plastic splinters are rigid and sharp enough to cause choking or injury if swallowed. Incident description: While eating, a plastic fragment from the fork broke off into my food. I swallowed it, choked, and vomited. After the incident, I inspected the remaining forks and found that all of them appear to be shedding similar plastic fragments. This presents a serious choking and ingestion hazard.

Injury information: Choking and vomiting after swallowing a plastic fragment. No medical care sought at the time, but the incident was distressing and dangerous.

Additional information: Product purchased at Costco. Packaging retained. Willing to provide photos and additional details if needed.
