The product, in the report: Green sippy cup with jungle animals on it manufactured by Munchkin product MCKTP 20 and there is a second number on a dial but I can’t read it. 7 oz cup made in China.
I purchased a munchkin brand sippy cup with a soft spout for my baby this summer at Target. [REDACTED] used the cup several times without an issue. Then one evening I noticed he was soaking wet and discovered that the spout of the sippy cup and had come completely off. Fortunately he spit out the tip of the cup and did not choke on it. The piece that came off is small and a true choking hazard. I still have the cup, top and spout at home. I also have a picture I can send in if you like.

Emergency room Made Jun 27, 2012, sent to the company Jul 6, 2012, published by CPSC Jul 27, 2012
The product, in the report: Munchkin Mighty Grip 8 oz. Trainer Cup, Blue cup with clear spout
A “Munchkin Brand” sippy cup, the “Mighty Grip 8 oz. Trainer” had the plastic/silicone spout on the lid rip while my 13 month old son was drinking from it. When the top ripped it allowed too much water to flow through at one time causing my son to choke on the water. His lips and tongue began to turn blue and he was given the infant heimlich maneuver to clear his lungs of the water. We called 911, but he was breathing and crying by this point. The 911 operator said that if he was crying he was getting enough air but we should still have him looked at by a doctor. He also sounded wheezy so we were concerned about that as well. We noticed that a piece of the cup’s inner spout was missing. We brought him to the Holland Hospital ER and he was given a chest x-ray. Although nothing showed on the x-ray the doctor suggest transferring him to Helen DeVos Children’s Hospital in Grand Rapids, MI. He was brought into the OR and given a bronchoscope where the surgeon found the small piece of the spout stuck in the main air way to my son’s left lung. We have medial documents and the surgeon also gave us the piece that was stuck in his lung.

The product, in the report: Munchkin Insulated Stainless Steel Sippy Cup
I have two Munchkin Insulated Stainless Steel sippy cups (they are less than one month old). As I was washing one of them (always hand washed, never dishwasher), the stainless steel liner came loose and then was able to be pulled out of the plastic outer shell. Inside the plastic outer shell, in the bottom of the cup, was a loose screw. This could have been a choking hazard if my 14 month old had been using the cup when it came apart.

The consumer says that the sippy cup has a built in straw. When her one year old drops the cup, the plastic piece used to push down the straw dislodges and she has found it in there mouth multiple times. She believes her children could choke on it. The consumer says this poses a choking hazard. The consumer contacted the manufacturer who told her there are no other complaints and didn’t provide her with any assistance.

The product, in the report: Munchkjn multi plate 5 pack, various colors BPA free States dishwasher safe top rack and microwave safe on the bottom of each plate
I bought a multi-pack of Munchkin BPA-free plastic plates a few days ago. I went to use one today, flipped it over to make sure it was microwave safe and noticed that it was clearly imprinted along the bottom rim that it was microwave safe. Well, clearly they are not as the plate caught fire in my microwave. I then looked online and found that others have had this issue. The earliest report I saw of these catching fire is from mid 2010! I don’t understand how the company has been able to mark these as safe for the microwave and cause these problems for a year and a half. They must have been made aware.
I do not have the packaging anymore, but I will include a link to the product on Amazon.
http://www.amazon.com/gp/product/B002ETXCEG?ie=UTF8&ref=aw_bottom_links&force-full-site=1
I have also emailed the company, but considering how long the issue has been allowed to continue, I can’t imagine they will do anything about it.
Also, I don’t know if my microwave is safe to use after having a fire in it. It wasn’t a giant raging fire as I was only feet away when all this happened (thankfully). I hope nonone else has to deal with this anytime soon.

My 22 months old child’s upper incisor pull out during a fall while drinking and holding the cup on the on a padded hardwood floor. The thin edge of straw open/close mechanism on the lid caught inbetween the gap of upper front incisors and the force from the fall levered out the entire incisor. This product should be recall.

The product, in the report: Mighty Grip Plastic sippy cup for babies and toddlers The bottom of the cup it says “10 oz, / 296 ml Made in China Copyright 2007 Munchkin, Inc.” Model Number 40040.
My husband, 21 month old daughter, and I were on the way home in the dark from shopping. My daughter said “Cup broke!” and we told her there was no way the cup could have broken. She was persistent so my husband turned on the light and sure enough she had ripped a portion of the spout off the Mighty Grip sippy cup. We could not find the piece and were worried she had swallowed it. When we got home my dear husband scoured the car and located it. My concern is that the torn piece was small enough to be a choking hazard as per the FDA brochure on choking and young children and I have no idea if it would have passed safely through her digestive system. I do have pictures I took when I got home.

Seen by a doctor or nurse Made Jul 31, 2011, published by CPSC Aug 19, 2011
The product, in the report: Munchkin “Mighty Grip” Sippy Cup with replaceable spout
Munchkin Brand, “Mighty Grip” sippy cup products using a replaceable plastic spout / bite valve. The method of flow control for the spout is an X cut in the plastic spout. A child bites on the spout to release flow of fluids in the sippy cup. After a few weeks of use, one of the corners of the plastic spout breaks, flow is uncontrolled, and unfortunately the child ingests the plastic particle.
I can offer photos of the defective product as wells as several actual failed spouts. Contact [redacted]. Many thanks.
