  1. No one hurt

Made Mar 11, 2013, sent to the company Mar 29, 2013, published by CPSC Apr 12, 2013

The product, in the report: Ultimate Chopper

Caller is contacting CPSC to alert the public to a safety hazard not covered with release# 05-258. His blade housing was replaced several years ago.

Around March 2nd, 2013 callers daughter was using the product, had toasted a piece of bread, cut into quarters and put into the chopper to create bread crumbs to place on a recipe she was preparing when the top of the blade assembly flew off around the locking mechanism, hit the plastic bowl with the unit still spinning causing plastic pieces to fly out. There are four pieces of plastic that are approximately 1 1/2 inch by 1 1/2 inch, with other sizes near that measurement but progressively smaller.

He is reporting because the original release does not imply pieces flying off the product which could cause laceration, eye damage etc.

    There were no injuries

He contacted the hotline for the original recall hotline and spoke with [REDACTED] who advised him that she would be happy to receive pictures that she would share with her supervisors, however they had blade assemblies, no bowls but she would conduct a search for him.

He is reporting because of the safety issues and laceration or eye damage hazard.

Made Oct 6, 2011, sent to the company Dec 16, 2011, published by CPSC Jan 10, 2012

The product, in the report: a chopper

While preparing to chop some bell peppers and onions she pressed down on the handle to start the chopping process.

When she went to release the handle the chopper it did not stop running so she tapped it again and that was when she noticed that sparks and smoke began to fly from the bottom of the chopper where the motor is located.

There was a electrical burning odor coming from the chopper so she immediately unplugged it from the wall outlet.

She has not used the chopper since the incident occurred and has not been contacted because she is unable to locate the number to the firm. She plans on contacting the retailer.
