The product, in the report: Progressive 9” Tower Aluminum Grater with black plastic sphere shaped cap with a triangular bottom (* Progressive International Corp. D438,433/6,135,375 Other Patents Pending) *Inscribed on the grater

I purchased a cheese grater with the following inscription: Progressive International Corp. (D 438,433/6,135,375 Other patents pending). I used it to grate cheese for an omelet. The handle became loose. While chewing, I bit down on something hard. When I examined it, it was a small screw with a washer attached. I tried to find out how this could have happened and subsequently discovered that the handle was screwed on from within the metal grater, unobservable to the used. Fortunately I was chewing carefully and slowly since I just had dental work done and so did not swallow it. This is a potential hazard since it is impossible to see that the screw was even missing. It took me looking over and over for where the screw might have come from before realizing it came from deep inside the tall metal grater.

The product, in the report: Listed on Amazon as “Progressive International Onion Chopper.” Model number GOC-300. Chops and dices onions. Consists of plastic container with a plastic attachments with metal blades in a grid. The item to be chopped is put on the grid, and a plastic cover is pushed down, pushing the chopped item into the plastic container part. Outside is clear and white, part with blades is green.

The chopper was used to chop onions into the smaller size. The onions were collected into the clear reservoir part and dumped into the pot. When I was eating the food, I realized I had a large, jagged piece of plastic in my mouth. Looking back at the chopper, I realized that the clear reservoir part had cracked and broken during use, and the broken plastic had fallen into the food.
