The product, in the report: Ozeri ZP-20gl 8 inch tempered glass fry pan lid purchased at HomeDeport.com for local store pick up Model ZP-20gl
An Ozeri 8 inch tempered glass fry pan lid explosively fractured when it came in contact with a damp dish wash cloth in our sink after being used to cover an egg being fried with steam used to poach the sunny side of the egg. I used it in this same manner just a few times before this with no issue. Some glass shards were blown into my fingers, but because the lid was down in the well of the kitchen sink the multitude of shards and fragments were contained within the sink.

First aid Made Feb 1, 2017, sent to the company Feb 15, 2017, published by CPSC Mar 9, 2017
The product, in the report: Tempered Glass frying pan lid
I was pan frying steak, I lifted the lid with my left hand so that I could flip the steaks over. I was going to turn the second steak when the lid that I was holding exploded. Glass flew everywhere, including into me. The only thing left in my hand was the lid handle, there are so many fibers/shards of glass in my skin. These are very dangerous lids. After the incident my husband typed in “Ozeri” exploding tempered glass and found that this has happened a lot.
