The product, in the report: Clear glass casserole dish with ribbed design around outside. Approx size 9 x13 or about 3 qt. size

The casserole dish was undisturbed but one side popped sending the glass flying off the counter and barely missing a child who was several feet away. The dish continued to crack & shatter sending shards of glass flying. The dish had a ribbed design around it and was approx. a 3qt., oblong casserole dish made by the Libbey company.
