  1. No one hurt

Made Sep 23, 2025, sent to the company Sep 26, 2025, published by CPSC Dec 2, 2025

The product, in the report: T-Fal - Initiatives Aluminum 6 Piece Cookware Set - Set includes - 1 piece 9.5’’ fry pan, 1 piece 2 quart saucepan with 1 piece lid, 1 piece 5 quart Dutch oven with 1 piece lid, 1 piece slotted spatula -

While cooking with a T-fal nonstick pot and its matching tempered glass lid, I experienced a sudden and alarming product failure. I was using the pot under normal stovetop conditions to prepare a fresh piece of fish when I heard a loud POP. The glass lid shattered around the handle area, and the knob detached completely, leaving a hole in the center of the lid. The rest of the glass remained intact but was visibly fractured.

I was the one using the product at the time. There were no sudden temperature changes, impacts, or misusethe cookware was being used exactly as intended.

Fortunately, no physical injuries occurred, but the incident posed a serious safety risk. The violent release of pressure and the detached knob could have caused burns or cuts if I had been closer. Additionally, I had to discard the expensive fish I was cooking due to the risk of glass contamination.

This experience was both dangerous and costly, and Im reporting it to raise awareness of a potential defect in the lids construction or mounting hardware.

The product, in the report: From Amazon: T-fal Ultimate Hard Anodized Nonstick Fry Pan Set 2 Piece, 10, 12 Inch, Oven Broiler Safe 400F, Cookware, Pots and Pans Set Non Stick, Kitchen Frying Pans, Cooking Skillets, Dishwasher Safe, Grey - https://www.amazon.com/dp/B07H18G2P4?ref=ppx_yo2ov_dt_b_fed_asin_title&th=1

Pan overheated rapidly after being on medium-high heat for 4 minutes. When raw vegetables were added to the pan, the food and the pan caught on fire. Although there was no property damage and no one was hurt, it was an extremely scary incident. If a child was nearby or if someone had been standing directly over the pan, there may have been severe injuries.

UPDATED ON 4/28/2025: [I just wanted to add that I can ship the pan to any inspector that wants to examine it or test it. I will keep it in my garage in case anyone wants to look at it / inspect it.]

The lid of my T-Fal Hard Anodized Nonstick blew up and shattered all over the counter and the floor. The lid was not on the stove and it had been off the heat for more than 30 minutes. The product, in the report: one T-fal pan is 12 in across and 2in deep. The other one is 12 in across and 3 in deep

I purchased T-fal frying pans from Walmart in Shelby Twp, MI. One about a year ago and the other one about 4 years ago. I was using them to fry eggs, brown hamburger etc with a gas stove. I noticed upon looking closely at them, that the middle emblem which is red with the letter T in it, is crumbling into the food, on both pans. When I took a picture of the two emblems, I also noticed that the one pan also has little yellow markings across the whole cooking area and they are all crumbling off as well. I’m not sure if we have injuries from this situation, as we have been internalizing this teflon and paint material for some time now. I notified T-fal company by email and they said this: Dear [REDACTED],

Thank you for your email via https://www.t-falusa.com.

Please note based on the pictures provided we could not determine a manufacturing related cause and thus it would not be subject to replacement under the terms of our warranty. This determination was reached by the discoloration from overheating. In most instances when the non-stick coating has lost its’ non-stick properties, it is due to overheating of the pan. Cookware with non-stick coatings should be used on low to medium heat settings, because due to the superior quality of our cookware, the cookware will heat up quicker and retain the heat longer. Overheating of the non-stick coating will cause the non-stick properties to break down. As a result, you will find that foods are sticking to the cookware. Another indication that your pan has been overheated is that your cookware has turned a brown or rusty color. If the pan has warped, buckled, flaking , cracked, rippled, layers have separated or there is some other gross defect, we would recommend not using the pan any longer. Overheating of the cookware is not covered under the terms of the warranty.

If you have any additional questions, please call 1-800-395-8325 and provide the consumer service agent with reference number [REDACTED]. Representatives are available from Monday to Friday 8:30 am to 5:00 pm EST.

    Sincerely,

    [REDACTED]

While cooking dinner in the Tfal 10.5 inch stainless steal pan, I heard and felt a bullet like explosion. My right ear lost hearing for about 2-3 minutes as I stepped back and turned off the heat immediately trying to assess the situation. About 4 feet away on the counter top I found a piece of metal shrapnel in the shape of a circle. After removing the food from the pan I found that the rivet had exploded off the inside of the pan by the handle. the remaining steal is extremely sharp and jets out into the pan where the rivet is no longer.

The product, in the report: Professional 12.5 Inch Fry Pan Numbers on the box: E938L894 2100081225

The consumer stated that on the box of the frying pan, in the front, the pan’s handle is advertised as “Oven safe up to 450F.”

The consumer stated that when he took out the pan out of the box, there was an insert in the pan. The back of the insert stated that “T-Sal cookware items and covers with plastic handles or knobs are oven safe up to 350F.”

The consumer contacted the mfr to get clarification on this matter. The representative informed him that the pan is safe to use in the oven up to only 350F. They informed him that the 450F on the front of the box is a typographical error.

The caller stated that if someone does not look at the insert and just go by the front of the box, they might misuse the product at a higher temperature than required for it to be safe to use in the oven.

The consumer added the frying pan did not come in a box, but was hanging from a hook by it’s handle and included cardboard packaging which was seen on both the front and back of the frying pan. The consumer noted he has included several photos of packaging. The consumer also noted the part of the packaging that was visible to consumers stated that the handle was oven safe to 450 degrees F, but when the packing opened, on the inside it stated that it was oven safe to 350 degrees F.

The consumer also added he has included a revised incident details to reflect the correction and have attached it to the original report. The consumer also noted he has retained the original packaging in case the actual packaging needs inspecting.

The product, in the report: T-Fal Ultimate hard anodized thermo-spot cookware.

I purchased a 12-piece set of T-fal Ultimate hard anodized cookware with the thermo-spot. From the first time I used them, I noticed they slid around on the burner more than I thought they should. So much so that you cannot stir in them or dish out food without holding onto the handle. One evening I put a pot of water on to bring to a boil for pasta. I was turned away from my stove preparing other things when I heard a noise. When I turned to look, the pan was wiggling (merely from the boiling water) and had slid most of the way off the burner and was headed towards the edge of the stove top. Had I not noticed this, it could have fallen off my stove and burned me. My main concern is the possibility of this happening when a child is in the kitchen. I do not believe they are safe to use because of this.

One other thing about these pots and pans is that the handles are held on with a peg and button-type top. Food collects in there and is difficult to wash out. If someone were not careful washing them, I wondered if tainted food left in there could contaminate new food being prepared.

I tried to return the product, but the store would not take them back. So, now I am stuck with this purchase.

I will also be writing to T-Fal about this problem, but doubt that anything can be done, because it wasn’t until after I purchased the product that I discovered it was made in China. From my research, I thought it was made in France.

Thank you for your consideration of this unsafe product, especially where children are concerned.

I was heating a flour tortilla in a T-Fal stainless steel fry pan when I heard a bang, almost like a gun shot, and something flew to my left.

I found a small round, size of a pea, piece of stainless steel in my floor. It was torn on one side. On inspection of my skillet I found that it apparantenly popped off the skillet where the handle is riveted on. My husband said it is a rivet cover

Thankfully the handle was pointed away from anyone. Someone could have been hurt seriously.

I did call your 800 number today and left a message so you will have a duplicate report.
