The product, in the report: Yard Waste container, distributed by Republic Services, manufactured by OTTO Environmental Services aka OTTO Industries, Inc
I incurred injuries while using an OTTO yard waste cart. My husband and I were working outside our home together, I on the driveway side of a short 3’ foot high fence, he on the other side of the fence in the yard. Our OTTO yard waste container, distributed by Republic Services, is located on the side of our driveway by our 3’ foot high fence, the opening facing the fence so when on the yard side of the fence we can easily open it for debris, yet pull it easily on the driveway side out to the street to be emptied. I had pulled the yard waste cart only 3-4 feet away from where it usually sits by the fence to trim row of bushes it sits by on the side of the driveway, and after filling it three quarters of the way full, attempted to push it the short distance of 3-4 feet back to the fence so my husband could reach it to put his yard debris from the other side of the fence in the cart. Leaving the lid open, as it was only going a very short distance, I pushed the cart from the hinge side towards the fence. It went about a foot, and then something stopped one of the wheels, dipping the hinge side towards the ground, and my momentum now going forward quite heavily caused me to step on the lid, causing the cart to now pull me towards the cart as it fell to the ground. My forehead hit the open top edge of the cart, opposite the hinge side, causing a 4.5 cm long laceration and 9mm deep on my forehead and a small laceration on my right eyelid.
