The product, in the report: Tripp-Lite 6-outlet power strip with phone line connection.

On 10/24/15 I arrived home from a brief shopping trip to find a very strong burning electrical odor in my home. Working smoke alarms had not activated, and there was no visible smoke, only strong odor. Upon investigation I found a plug strip had started a small fire which had self-extinguished. It burned/melted a hole in the carpet where it was placed. Black smoke had emanated from the first 4 outlets of the strip. The top back and one side of the strip melted/burned completely away. The plug strip was plugged into a standard grounded household outlet and was switched to the off position. It had one appliance (an iron) plugged into the device. The device is a Tripp-Lite 6-outlet plug strip with phone line receptacles, on/off switch, and indicator light. The UL label is visible on the device. It carries a sticker with the number 9918APTIP7542. Other identifying information is not visible, as that portion of the device was melted/burned away. It was rated for indoor use, which is where it was used. Fortunately the fire did not spread beyond the device itself and the carpet and pad on which it sat.

The product, in the report: Tripplite ISOBAR2-6 White metal housing S/N 2322AS0IB6936

I check to see that there was no power from a new Tripplite surge protector. As it costs as much to send it back as it does to buy a new one, I decided to take it apart to see what is wrong. I found that the wiring is disconnected on the inside and could potentially cause a short and thus a fire.
