The product, in the report: Belkin 6 outlet 8 ft 360 degree rotating plug surge protector
Surge protector outlet began arcing and melted with only 36w plugged into it. Product is Belkin 8 ft 6 outlet surge protector with 360 plug. There appears to be a recall for a similar product with this 360 plug from 2009. No injuries occurred. I began hearing a buzzing noise while my phone was plugged into a USB c charger that was plugged into this surge protector. I turned my lamp on (also plugged into the surge protector with a 10w LED bulb) and the lamp flickered at the same frequency as the noise. When I pulled the cord it had melted.

The product, in the report: Belkin Conserve Surge with Timer, white and grey in color
A structure fire in an executive office was believed to have been started by this surge protector when it failed. The product in question is the Belkin Conserve Surge with Timer. The product was plugged in appropriately and was reported to have never received any damages from normal use. It had one [REDACTED] charger (not connected to or charging anything), one computer monitor, and one desk lamp(found in the off position) plugged in to the timed shut off side and one monitor plugged into the constant power side. Due to the time that the fire started it is believed that the failure occurred when the timed shut off side of the surge protector was activated by the internal timing device. There were no injuries resulting from the fire. Over $100,000 in smoke and fire damages to the facility were incurred. This device was mass purchased for use on post. There are hundreds of this model with the same manufacture date on post. A second device was reported to have a similar issue months later but was deenergized before damages to life or property incurred. The second device had a desk lamp, two computer monitors and a laptop computer plugged into it. No property or persons were harmed by the second device as it started to overheat, smoke and make the sound of shorting out while the owner was present and was able to deenergize the device.

The product, in the report: Belkin Premiere SurgeMasterII
The use of a surge protector strip manufactured by Belkin (Premiere SurgeMaser II for DSS Systems). This device was used to connect my Smart TV, and the cable DVR cable box. I had turned my TV on and walked into the kitchen, the family room and kitchen is an open concept design. My sister-in-law walked through the family room (within minutes) and says she smells something burning. We then heard this weird noise, as I walked towards the TV I heard sparks coming from behind the TV. I immediately pulled the TV out from the wall and witness sparks and flames coming from the socket. I then turned the switch on the surge protector to the off position. With concern, the fire might still be burning inside the wall I called the fire department. They dismantled the socket and separated the wires once they had determined that the threat of the incident was no longer able to put us or our home at risk. After examining the surge protector, I notice that the prongs had completely burned off.

The product, in the report: Belkin SurgeMaster 6 port, 10 foot, w/ 360 deg. rotating plug.
Surge suppressor was plugged into the wall outlet, no other items plugged in to that duplex outlet. Small electric heater was plugged in and operating at a medium setting (1500W max heater). It had been running for approximately three hours when all of the sudden there was a flash and a “pop” and the heater and TV (plugged into a different duplex wall outlet on the same circuit) stopped working. A slight smoke odor was noticed coming from the wall outlet. The surge suppressor and the wall outlet were warm and had black soot.

The product, in the report: Belkin SurgeMaster Model F9M923-08 “Data Recovery Listed: E152555”
I was sitting in my bedroom and heard a buzzing static sound, looked over and saw my surge protector smoking. I rushed over to turn it off and it got worse. I moved my bed away from the wall and unplugged it. An electrical fire had started in the surge protector and turning off the switch on it didn’t stop it from buzzing and getting worse. There were no injuries, but I was very alarmed because if I hadn’t been sitting near it, it would have ignited the shag carpet it was sitting on and my house would have been burned down - or had it happened when I was asleep, it might have killed my wife and me. I WAS UNABLE TO SEND VIDEO DEMONSTRATING THE PROBLEM IN THIS REPORT SO I AM SENDING IT AS AN ATTACHMENT AND WILL INCLUDE CONTROL NUMBER IN THE EMAIL OMB Control Number [REDACTED]
I had sent this all to [REDACTED] at Belkin, and before sending him the surge protector I want to know if you want to see it first to inspect it. I am aware that corporations don’t always report dangerous defects to CPSC, so I’m initiating this report with you. I have communicated with [REDACTED].

No one hurt Made Jan 9, 2017, sent to the company Jan 27, 2017, published by CPSC Feb 10, 2017
The product, in the report: Belkin 6 outlet surge suppressor bar. Relocatable Power Taps. Transient Voltage Surge Suppressors. Model: BE106001-08R Listed: E152555 Maximum Load: 15A 125VAC 60Hz 1875W. Made in China. The rest of the information on the back I cannot read because it is covered in soot.
The Belkin surge suppressor, that we have had for years, just caught fire. We heard a buzzing noise first, and when we went into the room, there were flames, about a foot high, coming up from the surge suppressor. We unplugged the surge suppressor, put out the fire, and then examined the surge suppressor. The fire started on the back of the surge suppressor, there is a large blackened hole, along with electrical fire soot covering the suppressor and the power cords that were plugged in. The fire burned our rug quite severely. Luckily, the damage was limited to the rug. My [REDACTED], [REDACTED] and lamp, that were plugged in to the surge suppressor, were all working properly afterwards. We do not understand why this happened. Further, we do not understand why the electrical fire didn’t trip our breaker. As you can imagine, we were grateful to have been home, as the electrical fire, that started in the surge suppressor, could easily have burned our house down. I am sending this information to Belkin as well, as I do not know if this is an isolated incident, or if there is some kind of manufacturing defect in the surge suppressor. We are totally in shock, and now fearful, as we have 4 other Belkin surge suppressors, of the same type, in our house with electronic equipment plugged in. I am going to replace them with another brand as soon as possible. Please use this information to warn consumers - I believe Belkin should recall these surge suppressors.

The product, in the report: Surge Protector
The consumer indicates that immediately after she plugged in her surge protector for the first time, the unit started popping and sparking with small flames which went out after circuit breaker/GFE tripped.as the consumer unplugged the unit. The consumer states that the unit is black where the plug is located at. Nothing was ever plugged into the surge protector. The consumer also notes the outlet was burnt as well as where the surge protector was plugged in.
The consumer contacted the manufacturer and explained the situation. The representative she spoke to informed her that they’d make note of the incident. She asked if she should hire an Electrician and they told her to expect a call from them within twenty-four hours.
The consumer planned on keeping the unit for at least the next thirty days in case an investigation ensues. However, Belkin says she needs to send it [product] to them, and it has been sent to them on 5/31/16 via mail with tracking.
The consumer provided a letter, received on 6/15/16, stating:
Dear CPSC investigator:
Belkin telephone personnel noted my safety concerns and instructed me to return the outlet via their warranty program. I mailed the claim and surge protector via tracked US mail (tracking #[REDACTED]) on 5/3 1/16 to the address they provided. Their warranty description states that they will check the surge protector to ensure it is defective. Hopefully this occurs and Belkin will correct any design defects and/or recall any other similarly defective devices.
Attached is your report with my corrections and signature. The electrician said the outlet was not at fault. However, I still have it in case either Belkin or you need it. I am enclosing pictures as well as the electrician’s report. If you need me to send additional pictures or anything else feel free to contact me. I consider myself very fortunate that I was not injured. I am very grateful for the circuit breaker/GFE and luck that the sparks did not land on any papers or me!
Thank you for looking into this (hopefully) before anyone does get hurt.

The product, in the report: The surge protector is a 6 outlet Belkin BE106001-06R surge protector with listed number E152555 and a 360deg rotating plug.
We have our TV, DVD player, and streaming stick hooked up to a Belkin BE106001-06R surge protector with 360deg rotatable flat plug. The surge protector is plugged into a wall outlet behind a dresser.
While watching TV this morning, the TV and other items plugged into the surge protector suddenly shut off. I checked to see if the circuit breaker tripped, but it did not and all other lights, etc plugged into the same circuit still worked. I went under the dresser to flip the power switch on the Belkin surge protector on and off to see if the surge protector had a tripped breaker, and after switching it back on, there was a loud bang and flash which scared me and also tripped my home’s circuit breaker. When I jumped back in shock from the flash/bang I injured my hand on the bottom of the dresser the surge protector was under.
I removed the dresser and found the swivel plug portion of the surge protector had broken in the flash explosion and shorted the circuit out. Based on the manufacturer’s markings, this unit was made November 20th, 2014

The product, in the report: Belkin brand surge protector/power strip with a rotating plug that allows the cord to be positioned at any angle relative to the electrical socket in the wall.
Surge protector was plugged into outlet under my desk. I bumped my leg up against the power cord, and the power to my computer went off. I looked under the desk, and the surge protector was no longer plugged in. Looking more closely, I see the power plug had partially melted and two of the prongs had broken off and were stuck in the outlet.
The melting had apparently occurred earlier, as the plug was not warm. I did not feel the outlet to see if it was warm.
I see there is a recall on a different model surge protector from the same manufacturer (ref. http://www.cpsc.gov/en/recalls/2010/belkin-international-inc-recalls-surgemaster-surge-protectors-due-to-shock-hazard/). Though my failed surge protector is a different model, the “plug” looks identical to that of the above mentioned recall -- and it is the plug that failed in both case.
I have uploaded photos from my incident. You can view them here: [REDACTED]
Seems to me like any and all products using that sort of plug should have been recalled.

The product, in the report: Belkin BV108200-06R 8 Outlet Surge Protector with Rotating Plug
I purchased a Belkin BV108200-06R surge protector from Fry’s Electronics in June 2014, which I used for my computer and two monitors. This computer and the monitors, at peak, does not draw anywhere close to the rated maximum wattage of the Belkin surge protector.
On 1/31, the surge protector unexpectedly turned off and started to make a slight hissing noise. I immediately turned off the power to the room at the breaker box and went to remove the plug of the surge protector from the wall outlet, only to find that the plug was very warm and one of the prongs of the plug was loose. Upon further inspection, it appears that the plastic surrounding the prongs of the plug had melted.

I noticed my devices plugged into the Belkin Relocatable Power Taps, model number BV108200-06R-BLK, listed number: E152555 was not providing any power.
I also heard an electrical arc sound and took a look around for the source of the problem. The problem came from the 360 degree wall outlet plug where the plastic housing surrounding cracked, and the separation was the reason why there was no power.
I was fortunate enough to unplug it without getting shocked and had to temporarily relocate a better surge protector to run the equipment.

The product, in the report: Current Tap Surge Protective Device; plug-in type
We had a secretary at our Fire Department HQ who unplugged a Belkin Surge protector from a wall outlet in order to move it to another location. This particular model, #F9H620-CW, comes packaged with a longer screw meant to replace the short screw that keeps the protective cover plate over the outlet box.
When she removed the long screw and proceeded to pull the surge protector out of the outlet, the metal cover plate slid down onto the two metal spades of the surge protector as it was still making contact with the outlet.
There was an immediate flash, with minor, scorching damage to the device and the metal cover plate. Although she wasn’t hurt and no fire ensued, she was understandably shaken and recognized the potential for a more serious or tragic outcome. I work in the Code Enforcement side of our County’s Fire Marshal’s Office, and was asked to look into the matter. My first intentions were to see what - if any - info I could obtain about the product and how it was supposed to be used. Unfortunately, the original packaging had been discarded after the initial purchase, so I was unable to find out if the instructions indicated power to be shut off to the outlet prior to removing the protective cover plate from the intended outlet.
Although the product is UL listed and is manufactured by a large, established, well known consumer products company, I was unable to find any operating info on the manufacturer’s web page. Subsequent emails and phone calls to the customer service departments were less than fruitless. Their initial claim was that no instructions, literature or user information included with the product; in fact I was told there was no packaging at all.
I pointed out that I was looking the product on their web-site, and it was clearly packed in a clear, protective plastic sales/display arrangement you would typically see in any hardware or Home Depot/Lowes store. Also visible in the picture was the descriptive information about the product, though it is unreadable on line. After an extended ‘hold’, I was informed that the product’s packaging had no information regarding its installation or removal from an electrical outlet. Staff from the Investigation side of our office advised that this scenario has played out in fire calls in the past, and suggested that we forward information to your organization for possible further action. Having found no recalls showing on the manufacturer’s web page or the CPSC site, I request assistance from you in order to try to prevent further instances.

The product, in the report: Belkin Surge Suppressor Model # begins with F5C - remaining # was obliterated by fire
Due to a power outage, I had an emergency generator supplying power to my home. The natural gas furnace was running and most of the circuits were left off to keep the generator from being overloaded. My son was sitting in our family room reading, so I put the circuit which powers the receptacles and lights in that room on. The thermostat which controls the furnace reached its’ set point and shut off the fan and furnace. When the shut down took place, I heard the generator speed up for a period of time and then settle back down. WIthin a few minutes ( 5 to 10), I heard my son yelling ‘fire!’. The power strip next to the couch was producing black smoke and a small flame. I immediately shut down the generator and disconnected it from the house, then returned to the burning strip with a wet towel to put it under the smoldering strip to cool it down and get it off the carpet. I then unplugged it and removed it from the house putting it outside to help get rid of the smell and begin cleaning the soot from the carpet and surrounding area, There were 2 items plugged into the strip and neither of them was on. They were both lamps, one that had CFL and the other a low wattage lamp.

Listed: E152555 (UL listed holographic sticker; “40 YR”) No manufacturing code/dial present. Made in China
Intense electrical short within Belkin surge protector’s rotating male plug caused the plastic housing to melt and seperated the blade from internal connectors (metal blade is now at an awkward angle).
The following is what I reported to Belkin,USA, Customer Support:
Re: Relocatable Power Tap Surge Protective Device (Type 3) 15A - 125v Model: BE106001-08R QC#: J44B-C
This surge strip exhibited dangerous blade separation at the rotating male plug, which caused internal arcing and melting of the plastic housing (at the base if the blade).
The rotating plug head was never compromised nor mishandled in any manner. There was no power surge in the local utility lines as well.
I only had one item plugged into your device; one laptop. If the house breaker didn’t trip at the time of internal malfunction, my netbook would’ve been damaged beyond repair.
This item should be investigated for faulty manufacturing and similar complaints from other consumers...as THIS IS A FIRE HAZARD.

When we walked into our house Saturday night it smelled of something burning. It took me a few minutes to discover the source. It was a Belkin surge suppressor that caught fire.
We did have a power outage during the storm and still no phone/ cable tv/ internet. The power was restored when we retuned home.
If you have one of these Belkin surge suppressors, stop using it now. It did not handle the surge very well. It was not overloaded. Only 2 plugs were being used for a phone and alarm clock which are both functional. It could have burned the house down.

This is to state that on June 8, 2011 we had a house fire at our home. We were at the pond fishing and when we returned to the house I found the smoke detector buzzing as I went to open the carport door into the kitchen area. Black smoke engulfed me as I tried to enter into the kitchen area to see what or where the smoke was coming from. I then went outside to get my husband. We then realized our cocker spaniel dog was still somewhere in the house. As I went to the den area my husband went to the master bedroom, I found that the door to the computer room which is bedroom #3 to be hot. I then left the house. My husband retrieved the dog from under the bed in the master bedroom. We then called 911. The fire dept. came and put the fire out. The fire dept told us that the fire originated from the area of the surge protector ( the surge protector was cause of fire) Bedroom and bathroom #3, bedroom #2, and hall way were totally lost. The rest of the house was in complete smoke and soot damage. We have been at the [REDACTED] since June 8, 2011. We have lost all of our personal items along with the health hazard from this fire.
