The product, in the report: 32 foot Werner aluminum extension ladder
My son went up a ladder at a customers house. Came down the ladder went back up and it came slamming down, collapsing with my son on it. Thankfully he wasn’t permanently injured or killed.

The product, in the report: 20 ft aluminum extention ladder D1120-2
I was up.on the 20ft Aluminum D1120-2 extention ladder max weight allowed was 200lbs ( I was 125lbs) I had the ladder extended 17 feet up and was standing on the 6th rung from the top fly section all the sudden I heard a cling noise so I decided to get back off the ladder so I stepped down the next rung (7th) and the ladder started pulling away from the wall and was bending towards me. The ladder then tore apart and tore open the drywall on the home I was working at I then was flipped around with my foot between the rungs and the ladder we t down causing me to fall and broke my left talus bone in my foot my foot was turned the wrong way and arched like up on my tip toes, I ladder on the ladder with it broken on top half and my back against the wall. I hurt my back badly and cut my elbow it was very scary and I could have died. After years of surgeries I now have had to have part of my leg amputated in 2014. My career as a house painter is over and live off disability which isn’t enough to support myself.

I bought a Werner 32’ fiberglass extension ladder in 2019. I used it three times in total and was always garage kept, out of weather and UV light. The product, in the report: Dual Purpose Combination Ladder can be used as both step ladder and extension ladder
Description of incident with Werner DP6200 Dual Purpose 2-IN-1 Ladder (purchased 7/15/20 from Walmart.com)
I was about to step off the ladder onto my roof when the fiberglass rails at the extension pivot hinge buckled/shattered, collapsing the ladder and myself onto the concrete patio. I sustained a 2 in head laceration in the fall.
I was treated by my doctor and then by a doctor at an Urgent Care Center upon his recommendation.
I sent Werner pictures of the buckled/shattered fiberglass ladder rails and my open head injury.
Werner said they would refund the ladder cost if I signed a disclaimer of any responsibility on their part for the accident.

The product, in the report: Occurred on a Warner 32 ft extension ladder model number d1332-2 with date code of 121744J
When using the extension ladder the structural integrity of the last side failed causing me to fall approximately 10 feet. I went to have x-rays performed but they found no fractures and no brakes but since have experienced severe low back pain as well as neck pain since the fall since learning of the recall I’m now returning my ladder go home Depot . Something should have been done to let the public know this danger without having the research Werner ladder recalls on my own.

First aid Made Sep 17, 2018, sent to the company Sep 25, 2018, published by CPSC Oct 10, 2018
The product, in the report: Werner-MTIAA-Aluminum-22-ft-Reach-Type-1AA-375-lbs-Capacity-Telescoping-Multi-Position-Ladder from Lowe’s Home Improvement store
Hello, I just wanted to write you real quick. I recently purchased a Werner-MTIAA-Aluminum-22-ft-Reach-Type-1AA-375-lbs-Capacity-Telescoping-Multi-Position-Ladder from Lowe’s Home Improvement store. I paid $178 and was very excited to have a versatile utility ladder that could meet all my personal needs. I purchased it on July 25th, 2018; not two months yet. So far I’ve only used it for two things: getting into my attic through the man hole in our garage ceiling and for installing some directional antennas on the side of our house for our 4G wireless internet.
Today, I was very disappointed, a little scared, and injured on the ladder from Werner and can’t believe something like this would happen on a practically brand new ladder... I had to remove the antennas I had previously installed because we decided to go a different route with our internet services. So I brought the ole ladder around the house, opened it out all the way straight and ensured the clips locked in place, and then I extended the telescoping on each side equally to achieve the desired height I needed. I am always a little nervous on ladders because I’ve heard and read lots of horror stories, my brother in law actually had a close friend who passed away from falling off a ladder. So I made sure that everything was used properly on your ladder, everything was locked in place, and the C clamps from the telescoping part were secured inside the holes for my desired height.
I got almost to the top of the ladder as I began to feel something give and then completely release. The aluminum metal broke and bended at one of the ends where the telescoping part comes out. It’s kinda hard to explain through a message so I took pictures.
I was lucky that I was able to fall over to the right side where there was nothing in the way like the wooden fence on my left. I fell about 10-12ft from the ladder but I only came out with missing a small chunk of flesh from my right thumb as it caught on the metal while I grasped at the ladder trying not to fall. To be honest, I didn’t feel the missing chunk at all due to the adrenaline; it wasn’t until I realized I had gotten blood all over my pants that I looked and saw my thumb.
I feel relatively lucky that this is all the injury I sustained. Maybe I’ll feel a little bruised tomorrow but at least I’m not dead. Also, I do want you to know that I’m a 220lb 6’3” average sized male, I did not come close to the 375lb weight limit.

No one hurt Made Oct 26, 2017, sent to the company Mar 5, 2018, published by CPSC Mar 19, 2018
The product, in the report: Folding Attic Ladder with Metal Rails
I purchased this Werner Attic Ladder in 2006 and have had a couple of problems with the screws that hold the steps that have broken. I have notified Werner once and the sent me a replacement. I replaced it and since then have had the same problem a few times. The screws that hold the steps will suddenly just break and cause you to slip of the ladder. I have been lucky as the times iot broke it was the bottom 2 o3 steps so I caught my balance. If it happened on the higher steps I could have sustained a much more serious accident. It is Rated at 300#’s and I am close to that at 275#’s but that should happen. They should have a recall on these or at least a fix that would make them safer.

We own a Werner the Equalizer Model D1724-2EQ MK 1 Type II 24’, it was bought at Home Depot Approximately 3 Years ago for $250. On October 7, 2013 my Husband went to use this Ladder to do some repairs on Our Home. He was in the process of Extending the Ladder with the Rope that came with the Ladder, it was approximately 20’ extended when the Rope just snapped and broke, sending the Extension suddenly back down toward my Husband, He incurred bruising and swelling on both hands, Thank God no broken bones incurred. On October 9, 2013 we contacted the Werner Co. told the situation asked them what the rope was made out of. On October 14, 2013 they responded back that they needed photo’s of the Ladder and damaged Rope, they also said that the Rope was 5/16” Polypropylene,we sent the photo’s per their request. They requested our shipping address so they could send us a new rope, but we had gone out and purchased a new rope. they stated that their Engineers would look at the photos and get back to us, as of this date no answers yet. We sent them another e-mail stating that we would like to be compensated for my Husband Pain and Suffering, we would like to be reimbursed the $250 that we paid for the Ladder. On October 16, 2013 they responded back that they needed what were His injuries, Doctor/Hospital Bills, Our Address, Date of Birth, Social Security Number, in order to file a claim. We are not giving out our Social Security Number, we don’t have Hospital/Doctor bills because nothing broke, Only Bruising and Swelling of the Hands. The bottom hinge is between 3-4 feet from the ground. The ladder is a tri-fold ladder which extends to a whole ladder. The consumer isnt sure if his mother was seen by a doctor, but would assume she has.
The consumer’s mother fell from landing on the floor tiles below while striking her shoulder on a freezer, resulting in a sprained ankle, bruised shoulder and elbow. The consumers mother is 5’1” and weighs 141 pounds. The ladder is intended to hold 300 pounds. The consumer’s mother was not carrying anything on her person at the time.
The consumer has seen and excess of 50 reports on these products online. The consumer called the firm about the yesterday but they were not available and has not been able to speak with anyone at the firm. The consumer called the retailer yesterday and was informed to bring the ladder to them and they’ll see how they may help him.

The product, in the report: Werner Type I-A (300 Ibs rating) 32-foot fiberglass extension ladder
On March 30, 2010, I purchased a Werner fiberglass Type IA 32-foot extension ladders from my local Home Depot. It’s rated at 300 Ibs (I weigh 260), Model No. Model D6232-2 on the Home Depot web site. While unloading yesterday I noted it has developed what I would call “stress fractures” where the upper three Drungs attach to the first (bottom) rail section. I question whether the ladder is now safe to operate fully extended. This is especially troubling with a ladder only two years old. Frankly, my business has not been all that great in that time either, so I would estimate the ladder has been used fully extended less than 50 times! I am attaching photos of the cracks and a copy of my original receipt for the purchase. and I would submit that an investigation and possible safety recall should be considered before someone gets hurt, if it hasn’t happened already.

The product, in the report: Werner Equalizer adjustable ladder legs for uneven surfaces.
I was using Werner D1700 - 2EQ Series ladder with adjustable legs when I felt it to be unstable while I was about 14 feet off the ground. I returned to the ground and checked the adjustment of the right leg. I found that two of the four bolts that held the adjustable leg to the ladder had broken in half causing the leg to be very loose. I am very lucky to find this before the other bolts broken. The legs adjust with a pin not the bolts so I can’t understand why they would break unless they were made to soft to handle the load. This ladder is only a few weeks old.
