The product, in the report: Little Giant leveler Type 1A Ladder – adjustable ladder

Attorney representing client stated that the Little Giant Leveler Type 1A Ladder (adjustable ladder) made exclusively for [REDACTED] failed unexpectedly similar to the recalled ladder failures involved in CPSC Recall #17-113 dated 03/23/2017. The ladder’s rapid lock fasteners failed after the client adjusted them when suddenly the ladder collapsed while the client was on the ladder in August 2021 and was preparing to adjust the video camera’s in his wife’s store. The client sustained a fracture to his left wrist and required surgery that included pins and screws to repair his injury. The client also sustained contusions to his leg. The attorney hired an expert who tested the ladder and revealed that all of the rapid lock fasteners failed on the entire incident unit. The attorney has detailed photographs and a video of the incident. The client has limited use of his left hand and wrist.

The product, in the report: Little Giant Folding Ladder

I was using a Little Giant Ladder Type 1A ,hinged folding ladder opened strait and extended to 11 ft.The right leg was extended a additional 8 in. over a step using a Little Giant Leg Leveler attachment. While I was standing on the ladder the central ladder hinges suddenly failed, allowing the top section of the ladder to rotate left and the bottom half to rotate right! The ladder fell to the deck surface with me landing on the ladder and deck from a height of about 9 ft.

I consider myself lucky that my injures were limited to bruising, swelling and lacerations, but no bone fractures! The hinge locks worked ,but both hinges show severe torsional distortion and mechanical damage. The ladder was set up and being used properly, and I weigh 240 lbs which is well within the 300 lb. ladder rating. This mechanical failure occurred without warning and should not have been possible,I want to know how and why this ladder failed!

The product, in the report: All info is Little giant 15413 13 foot type IA

I was working in my shop hanging electrical wire. I was up on the 5th step with the ladder extended to 13 feet. it was leaning against the wall at a slight angle. As I was reaching to fasten the wire the ladder secure pins failed and it dropped down as I went down my feet slipped off that step and my legs slid along the step till I stopped on the next step and my shins were both bleeding through my levis. I was not sure why the ladder failed. I had bruised shin bones and flesh wounds that took a long time to heel. Little giant ladder type IA 13 foot # 15413. Date code 10462P
