  1. No one hurt

Made Jul 27, 2026, sent to the company Aug 5, 2026, published by CPSC Aug 19, 2026

The product, in the report: PRODUCT IDENTIFICATION Product - Adjustable bed base, powered, split California King configuration. Brand - Tempur-Pedic. Model - TEMPUR-Ergo ProSmart Adjustable Base, retailer item no. 143786P. Two units. Manufacturer - Tempur Sealy International, Inc., 1000 Tempur Way, Lexington, KY 40511. Mattresses in use on the bases - TEMPUR-LuxeBreeze Medium Hybrid 13 inches, split California King, retailer item no. 143781P. Two units. Retailer - Mattress Firm, store number 501076, 5414 Crossings Drive, Suite 101, Rocklin, California 95677. Date purchased - May 16, 2026. Date delivered and installed by retailer - July 18, 2026. Product still in my possession - Yes. Removed from service. INCIDENT DESCRIPTION Two powered adjustable bases were installed side by side in a split California King configuration, each supporting its own split California King mattress. The two bases articulate independently. When one base is raised or lowered while the other remains stationary, the two mattresses move relative to one another and the gap between them opens and closes. As delivered and installed, with both bases flat and level, the gap between the two mattresses measures approximately four inches and runs the length of the sleeping surface. This is the condition of the product at rest. It is not a product of articulation. A split California King consists of two halves that are intended to meet and form a single seventy-two inch sleeping surface. These do not meet. On July 21, 2026, three days after the retailer installed the system, the arm of my six-year-old son became lodged in the gap between the two mattresses while he was on the bed. It did not come free on its own and I had to extract it. He was not injured and required no treatment. The bases were not actuated while his arm was trapped. Had either base been actuated at that moment, whether by a control input from him, from another occupant of the bed, or inadvertently, his arm would have been drawn into the closing space between the two mattresses and the moving base frames beneath them. The mechanism is powered and is operated by handheld and application-based controls that a child can reach and activate. The hazard is that a split adjustable base configuration creates an accessible, powered pinch and shear point running the length of the bed, at a height and location readily reachable by a small child, with no barrier, guard, obstruction sensor, or interlock preventing access to it or arresting motion when something is in the gap. For comparison, and without contending that either standard governs this product, the FDA Hospital Bed System Dimensional and Assessment Guidance recommends that openings in the identified head-entrapment zones be less than 120 mm, or 4.75 inches, and identifies 2.375 inches as the neck-entrapment dimension. CPSC adopted the same 120 mm dimension in its safety standard for adult portable bed rails, where it represents the head breadth of a fifth percentile adult female. A four-inch gap is nearly double the neck-entrapment dimension, and the head dimension is calibrated to an adult rather than a young child. Both standards address static gaps. This gap closes under motor power. So far as I am aware, no mandatory federal standard addresses gap dimensions or obstruction detection in consumer adjustable bed bases sold in split configurations. The industry appears to recognize the hazard. The Sleep Number published support guidance for split king mattress separation instructs owners that if the gap creates a safety concern, or if a child, pet, or sleeper could get caught in the space, the owner should contact support before using the bed again. The Tempur-Pedic published owner manuals for the TEMPUR-Ergo line state, verbatim, CAUTION - Keep children away from electrically adjustable beds and control units. There is risk of injury and electric shock. That warning does not identify limb entrapment in the mattress gap as the mechanism. I do not know whether the installation crew for the retailer fitted the mattress retainer hardware specified for this configuration. I have asked the retailer and the manufacturer to confirm what hardware was specified and what was installed. HARM No injury occurred and no treatment was required. This report is submitted on the basis of an actual entrapment event and the risk of injury it demonstrates. The foreseeable injury is a crush or shear injury to the hand, arm, or head of a young child. ACTION TAKEN The system has been removed from service. I have notified the retailer and the manufacturer in writing of the entrapment and the hazard.

The product, in the report: Adjustable Mattress Base

We recently purchased a new mattress and base from Matress Firm. The two speakers for the lower part of the bed that operate as a massage unit are both vibrating loudly, are hot to the touch after just a few seconds of running smell like burnt electrical wiring. I think this bed may be a fire danger and is clearly malfunctioning.

I notified Mattress Firm right after I arrived home the day of the delivery and realized that something was wrong with the base. Mattress Firm initially responded by asking for more detail which I responded back to them with. But since then they have not has responded to any of my requeststo remedy this problem so unfortunately I have to address this outside of their company to see what avenues we have to resolve this matter.

In the meantime we have a potential liability for them and for us in our home and smelling of burnt electrical. I dont know what to do.

The product, in the report: Motorized adjustable bed base.

I had unplugged my Tempurpedic Ergo Premier adjustable base for about 30 minutes while I rearranged my bedroom. When I plugged it back in I heard a pop, I tested the bed and it worked fine and then left the room. I came back about 10 minutes late and immediately noticed the room was Smokey I checked the bed power supply and saw smoke pouring out of it and just as I unplugged it, there was a flame.

The product, in the report: Vibrating Adjustable King Sized Bed w/Two Twin Sized Mattresses

Mattress w/massage feature and head/foot raising feature purchased around February 24th, 2013 but delivered March 4th, 2013

March 5th, 2013 was the first time the caller used the massage feature she believes that she accidently turned it to the highest setting.

Vibration should stop automatically after 30 minutes and the head area did just that but the foot area did not shut off, she couldn’t shut it off with the controller so she got up and unplugged it to cut power.

She did not plug it back in but when she returned to bed after unplugging the power to the vibration she states that her body was still vibrating, she could not sleep because of the shaking in her body.

She went to a different bed in her house in the attempt to stop shaking, however it did not stop and it continued through the next day whether she was sitting or lying down or moving around.

She has been to her physician twice because she hasn’t been able to sleep because of these tremors. The doctor on the initial visit prescribed sleep medicine she is waiting for the referral from her physician to enable to be seen by neurologist.

A contract signed at purchases says the bases are non-refundable. They were $3000 each.

She did contact the store who advised her that the vibrating bases are not returnable. Total cost of mattress and bases were $9700
