The product, in the report: Beds by George, Haven series. Material type blue canopy with [REDACTED]. Aluminum metal frame for the canopy. Brown wooden base bed frame. Mattress.
10 year old was left in a Beds by George bed with canopy. The canopy is supposed to be attached to a metal frame and the metal frame is supposed to be secured to the wooden base bed frame. The metal frame, at some point, was no longer attached to the wooden frame and the 10 year old girl was able to put her heard/neck in between the frames and became pinned. She asphyxiated.
