  1. No one hurt

The product, in the report: Rolling office Chair

I have a office chair at my home. The brand is true innovations and the model is 41832 with ITM. 633882. While I was leaning back on it 2 of the bolts that secure the upper cushion to the chair sheared about a 1/4 inch below the bolt head. There are 4 bolt that hold the upper to the lower. The 2 with the greatest amount of tension snapped. The shear appears to a almost smooth shear except for one sharp edge that appears to be where the bolt thread was. I have contacted the manufacturer and I am waiting for a response.

The product, in the report: chair with a polyurethane foam faux leather

We bought a chair from OfficeDepot on Markham 2-3 months back and on the particular day a foot long nail came out of the chair bottom cushion and hurt the underside of my thigh. The injury was not serious but has the potential to be serious in someone else. Please see the right hand corner of bottom cushion in the pictures. We have removed the nail and it is about foot long.
