Reports people made to the U.S. Consumer Product Safety Commission. CPSC publishes each one after sending it to the company named, and doesn’t guarantee they’re accurate. A report isn’t a recall.

Each report is the words of the person who made it, as CPSC published them. The company’s comments, where CPSC published any, are set apart as the company’s own.

Made Feb 28, 2015, sent to the company Mar 13, 2015, published by CPSC Mar 27, 2015

The product, in the report: RS To•Go™ Matrix Armless Task Chair, 40 1/4”H x 24 3/4”W x 28 1/4”D, Black Frame, Black Fabric Item # 93760 by the Swinton Avenue Trading Ltd., Inc.

The base shattered when my boyfriend was sitting in the chair ... he leaned back and ended up on the floor bleeding after the plastic base that was holding the caster on, broke in pieces and punctured his leg behind his knee on the way to the floor.

  2. First aid

Made Jun 26, 2014, sent to the company Jul 7, 2014, published by CPSC Jul 21, 2014

The product, in the report: Realspace® PRO 3000 Series Custom Fit Fabric Mid-Back Chair, Black/Black. Completely adjustable for added comfort •Fully adjustable back raises, lowers and tilts for optimal comfort. •Overall dimensions: 40 1/2”H x 25 1/4”W x 22”D •Tilt controls for ideal positioning. Adjustable tilt angle with infinite tilt lock lets you lock the chair in the most comfortable, angled position. Adjustable forward tilt control helps add comfort by reducing disc pressure. •Recommended for moderate use 3-6 hours per day. Curved backrest and seat cradle your body for hours of comfortable seating. •Height- and width-adjustable arms help relieve shoulder strain. •Effortless 1-touch seat-height adjustment. •Waterfall seat cushion slopes slightly downward to help reduce stress on your legs. •Dual-wheel casters let you roll from 1 task to the next. •This chair design meets or exceeds ANSI/BIFMA performance standards. •Weight capacity tested to support up to 250 lb. •Easy assembly instructions included.

The chair was used less than two hours when the armrest screws stripped out creating a pinching point for fingers and arms. These armrests are not repairable. The chair was listed on the Office Depot website as a drafting chair/stool. The maximum seat height is 40.5 inches. The seat and tilt adjustments do not stay in their locked positions thus causing the user to fall downward or tilt abruptly.

Overpriced for such a poorly made chair. I would NOT recommend! The chair is inherently dangerous.

    The product, in the report: Chair: Realspace PRO™ Quantum 60% Recycled Mesh Mid-Back Task Chair, 50 1/2”H x 29 1/2”W x 28”D, Black Frame, Gray Fabric (234750)

The item involved was a computer/office chair, sold by Office Depot, ordered online 29 December, 2010 and placed into service within the first fortnight of 2011.

The item is described in the order confirmation email as: Realspace PRO™ Quantum 60% Recycled Mesh Mid-Back Task Chair, 50 1/2”H x 29 1/2”W x 28”D, Black Frame, Gray Fabric (234750)

The seat back connects to the seat bottom by using two mushroom-head bolts to connect metal tongues attached to each. The failure mode appears to be the heads snapping from the shafts because of metal fatigue. When the heads snapped off (the first snapped off with a “pop” the prior evening but I could not determine what actually had happened) there was nothing to maintain the connection, the seat back fell suddenly and I fell with it, bruising my left tricep I possess the second head which was trapped under the seat back, the other is somewhere among my staged recycling. I also possess the bolt shafts and a section of plastic molding snapped off the seat back by the seat bottom’s tongue. In the design the bolt heads appear to be subject to significant, repetitive stress as the seat back flexes during normal posture changes.

    The product, in the report: OFFICE CHAIR Realspace® PRO Quantum Mesh Mid-Back Task Chair, 43 3/4”H x 29 1/2”W x 28”D, Black

Dear Mr/ Ms. I bought “Realspace PRO Quantum Mesh Mid-Back Task Chair “for 2 to 3 years at OfficeDepot. Last year, the chair back broke off from the chair base. There were just 2 simple screws attaching chair back to its base. And those screws sheared off because they are not even “metal”. As an important items of design, those screws were supposed to be steel, but they were just alloy. I have contacted the manufacturer and they send me a “repair kit” which includes 2 simple screws again. After fixing it ,I have been using chair for another year.

On this Tuesday ,I was working on the computer, when I leaned back, the chair back broke off from the chair base again. I fell back and hit my spine at the edge of chair back. I am still having pain when I am walking. I am going to see my doctor if it does not get better in a few days. The repair kit did not work. The plastic sleeve at the bottom of the chair back was torn and entire back fell off.Alloy screws sheared off again. This chair is a dangerous defective product. After doing online researches ,I have seen that people were having same problems about this chair.People contacts Office Depot for the same reaoson but they still sell this product .I think it is because ,the pay out to people is too small compared to earnings received from this chair. The reason I contact you is that I would like to inform you that “repair kit” does not work. I’d also like you to contact manufacturer or Office depot to address this design flow before any other customer hurt themselves. Sincerely, [REDACTED]

The product, in the report: Mesh fabric and gel-padded armrests let you work in comfort •Mesh breathes to keep you cool and looks good, too. •Height-/width-adjustable armrests with unique gel padding helps relieve shoulder strain. •Seat cushion glides forward or backward to best suit the length of your legs. •Change the tilt with the touch of a finger! Synchro tilt reclines the backrest at a 2:1 ratio to the seat, increasing the angle between your torso and thighs for a comfortable, ergonomic sitting position. Adjustable tilt angle moves both the seat and backrest at the same ratio, letting you rock or recline in comfort. You can also adjust the angle of the back cushion and the seat. •Effortless 1-touch seat-height adjustment. •Adjustable lumbar cushioning helps support your lower back and enhance the natural curve of your spine. •Waterfall seat cushion slopes slightly downward to help reduce stress on your legs. •Steel frame construction ensures durability. •Designed for intensive use (5+ hours per day). •Easy assembly instructions included. •Greener choice — contains 35% postconsumer recycled content.

Though extremely comfortable, my office chair (Realspace Quantum PRO™9000) failed at it’s only job of being a chair. I’m 165 lbs and 5’10”, so it should be able to support me since it’s advertised to hold up to 225 lbs (http://www.officedepot.com/a/products/510830/Realspace-PRO-Quantum-Mesh-Mid-Back/#tab2).

I sat down and the back of the chair fell off causing me to fall backwards twisting my back. The screws that affix the back of the chair to the seat completely snapped in two. I also noticed an absence of anything resembling a quality control stamp. I see that the manufacturer is offering a repair kit (http://www.quantumchair.com/recall/ordform1.html) which I assume simply consist of stronger replacement screws. The fact is that the plastic bezel that holds the screws in place is now cracked. A total waste of $300.
