Quantum Realspace PRO™ 9000 Series Mid-Back Multifunction Mesh Chair from CPSC Release # 10-013. Used for light duty home office work. Received and installed the recall replacement bolts around 2016 after the back fell off with the original hardware. On December 7, 2023, one bolt had unknowingly snapped and fell out of chair without provocation. I leaned back in the chair and the second bolt had also snapped, causing the back to fall off. My cat-like reflexes thankfully saved me from injury. This time.

The product, in the report: High backed office chair

BACKGROUND: The Quantum PRO 9000 office chair was recalled @ 2009. According to the recall, chairs should be taken out of service until a repair kit is received. New bolts are to be installed and the problem will be fixed.

THE PROBLEM: The chair was designed to have FOUR screws hold the chair back in place. Only TWO holes are provided in the chair back. When you receive a replacement kit, you replace the TWO screws with a different type of screw. However, there are still only TWO.

THE INCIDENT: Because there is too much pressure on these two screws, they are beginning to break. The heads of the two screws in one of our chair backs completely sheared off, nearly causing the employee to fall to the ground. No damage done this time, but definitely a potential hazard.

THE VENDOR: I contacted Raynor at the number listed on the recall notice, and they are sending me a screw extractor and a kit to install TWO new screws.

RECOMMENDATION: 2-3 years ago, one FAA office received all new chair backs, which contained FOUR screw holes in the chair back and FOUR screws. I believe this to be the proper solution for a chair designed to have FOUR screws holding the back in place.

NOTE: Our office currently has 20 of these chairs in use.

The product, in the report: Quantum Realspace PRO™ 9000 Series Mesh Chair with Headrest SKU # 690690

On 20 March 2009, I purchased Quantum Realspace PRO™ 9000 Series Mesh Chair with Headrest SKU # 690690

Wasn’t notified of the recall until today, 4 July 2015, which is over 5 years and 8 months after the recall was issued.

On 16 February 2013, one of the bolts broke (I guess that loosens one half of the bolt from the other half), when I leaned back in my chair, but happily I have quick reflexes and leaned forward and jumped up as I felt the back begin to sag and heard the bolt head hit the floor.

Since half the bolt was still in the chair, it took me some time to figure out how to remove that half, before I could replace it with the spare bolt from the parts kit that came with the chair when I purchased it.

After talking to a number of people, I tried [REDACTED] the head back on the bolt hoping that would help remove the broken part, but the glue didn’t hold tight enough. Someone suggested drilling a hole and using an [REDACTED], but my first attempt to drill the bolt out failed, and dulled a standard drill bit. I had to purchase a cobalt drill bit and an [REDACTED] to complete the removal. All told I spent about 5 hours over 3 days learning how to do the repair, and performing the repair attempts, which were finally successful. Once the broken half of the bolt was removed, the replacement and reassembly of the chair was straightforward.

The product, in the report: Quantum Realspace PRO™ 9000 Series Mid-Back Multifunction Mesh Chair

I am sitting in my chair talking on the phone. I am relaxing and leaning back slightly in my chair. The back of my chair snaps off. I fall backwards. My left arm lands on/in my trash can, and I crash to the floor with my legs still up on the seat of the chair.

The product, in the report: The chair listed here with the extra neck support http://www.quantumchair.com/recall/cpsc.html

I own the chair listed here with the headrest and actually leaned back in it today and the screws holding the top and bottom together popped out and the plastic broke off. The top of the chair was leaning all the way back on the floor and I would have fallen backward and hit the ground if I hadn’t grabbed the arms of the chair in time. I’m just glad there was a recall and I get a kit to fix it. I’m equally glad I stumbled upon it while researching a new chair to buy.

realspace pro Quantum Series Item 234-750/510-830 My husband was sitting in the chair. When he stood up, it fell apart; the back separated from the seat. Because he stood up, he was not injured, but he wants to get it repaired as it was extremely confortable prior to this incident. I have the same chair, but have had no incident this far.
