Each report is the words of the person who made it, as CPSC published them. The company’s comments, where CPSC published any, are set apart as the company’s own.

The product, in the report: The chair that we have is one of the broken chairs and bore an Office Master Inc. label Reg. No. CA-28684; and a product number 5215759; It appears to BC 48 Task Series of Office Master’s BC Collection

While a lady was working at an office in Chico, California and went to sit down in her office chair, the chair seat broke off of the pedestal, and she fell on the pedestal and then to the legs of the chair and the floor injuring her buttocks and lower back. She suffered nerve damage that persists five years after the injury, and has had to charge occupation so she could alternate between sitting and standing during the workday. The chair was sold by Office Master Inc. who imported the chair from the manufacturer, King Hong Industrial Company Ltd. Of Taiwan. A second chair of the same manufacturer and model failed several weeks later and the person that fell when that chair broke sustained unknown injuries. I am filing this report because it appears the chair failed from normal use and that the chair should not have so failed.
