  1. Hurt, no treatment

The product, in the report: Quantum Black Mesh Mid Back Chair SKU 510-830

I have a Quantum Black Mesh Mid Back Chair (SKU #510-830) purchased from Office Depot in 2009. The vertical back is attached to the seat base by two allen-wrench-headed bolts. I found a sheared-off bolt head lying on the floor recently and hunted around but could not identify where it came from. A month later, while I was leaning back in the chair, the second bolt also sheared off. The chair back fell off and with no warning at all I fell backwards off the chair.

I was (fortunately) sitting at a desk. My knees hit the underside of the desk as I fell backwards, stopping my fall. My back is really hurt but it kept me from falling and splitting my head on the floor.

The product, in the report: Black Executive Task Chair, Faux Leather.

I purchased this chair from [REDACTED] around 2012 and have had nothing but problems with it. The primary thing is that it rocks to the left and right, and sometimes forward and backward. It is not solidly on the base, which has caused me to fall sideways and backwards four separate times. I hadn’t thought about reporting anything as I wasn’t aware of this website and hadn’t seen any defective product reports on this product. The last date I fell from this chair was approximately in July of 2015 last summer. This fall caused two people to fall as when I fell backwards, it pushed the person behind me backwards as well causing her to fall, which caused injury to her leg. As I already have a neck injury, I do not need a chair that is this unstable. This has been an ongoing problem with this chair.

Caller stated that she experienced similar problems of recall#14-188. Hazard:The mounting plate weld can break and separate the seat from the base of the chair, posing a fall hazard to consumers.

She stated that the chair malfunctioned while in in use but she did not fall.

The manufacturer was not contacted as yet.

Caller feels that her model should be recalled as well because of the safety hazard it poses.

The product, in the report: Office chair: Quantum Mesh Chair with Headrest

A chair broke causing injury to myself and I Googled for it and found out it had been recalled which was news to me. Reading the recall notice, it says that “bolts attaching the seatback on the recalled chairs can loosen and detach, posing a fall and injury hazard to consumers”. This misstates the problem entirely. The real problem is not that bolts loosen and detach, the problem is that the bolts fail under tension load. That means that tightening them may make the problem MORE likely to occur and one may think that tight bolts would mean that they are safe. The bolts supplied in the seat back attachment are made from pot metal and not hardened steel. The failure mode occurred with bolts fully tight. The recall notice must state that the problem is that the bolts are not strong enough for the task and NOT say that the bolts may come loose. A thinking person may be mislead to believe that he is safe if the bolts are tight and this is not the case. Please have them change this language and keep them from misstating the facts. I have a neck injury as the result of two snapped bolts causing a fall against a filing cabinet. See below for what I have pasted from the release that I found on line:

The product, in the report: Black Mesh Office Chair

I purchased 2 Quantum Mesh Chairs from Office Depot (SKU # 510-830) for around $200.00 dollars a piece that have broken and caused a fall. So far I have experienced 3 major defects in the product over time so the “Incident Date” is only an estimate. 1) The screws holding the back cushion of the chair broke sending me backwards towards the floor. It also cracked the plastic cover at the base of the back cushion in the process leaving exposed sharp edges. 2) The casting on the left hand arm rest broke at the attach point and the arm rest fell off when I was getting up out of the chair but I caught myself - no incident. 3) Upon further examination of the chair I noticed the base casting for the seat frame has cracked towards the front of the chair by the tension spring - no incident.

I have detail pictures I could forward if needed.

The product, in the report: Realspace® PRO™ 9000 Series Mid-Back Multifunction Mesh Chair With Head Rest, 51 3/4H x 29 1/2W x 28D, Black sold by Office Depot

Realspace PRO 9000 office chair sold by Office Depot. The back of the chair is attached using two bolts. Today (after 2 1/2 years of owning the chair), I leaned back in the chair, and the two bolts snapped in half.

I fell back onto the back part of the chair (my head just missed a table behind me). I have a scrape on the bottom part of my back (about 2x2”), and a 1/4” deep wound near my right should blade (from the bottom part of the chair back -- where the screws attach). I also have bruising and soreness around my shoulder blade, and my lower back does not feel good (feels like I pulled a muscle, but will have to wait until morning to know for sure).

I am 174 lbs, 5’9”.. so I feel this chair (which was $350, and has a 10 year warranty) should not have failed in this way after only 2 1/2 years.
